This role involves developing and writing support plans, handling annual meetings, tracking goals and outcomes, providing guidance to house managers, and ensuring compliance with regulatory standards while fostering a positive environment for individuals with disabilities. The ideal candidate will demonstrate strong leadership, exceptional relationship management skills, and a passion for social work. As a key figure in our team, you will drive program excellence, support behavioral health initiatives, and ensure administrative efficiency to deliver impactful services that enhance the lives of those we serve. Responsibilities Lead and supervise staff to ensure high-quality service delivery aligned with the individuals support plans and regulatory requirements Develop, implement, and evaluate plans and programs tailored to meet the needs of individuals with developmental disabilities Build and maintain strong relationships with persons served, families, community partners, and stakeholders to promote collaborative support networks Monitor behavioral health interventions and support strategies to promote positive outcomes for clients. Track and report incidents at required by 245D Ensure adherence to all licensing, safety standards, and organizational policies through regular audits and staff training Requirements 3-5 years of experience in developing and writing support plans, positive support transition plans and person centered planning. Proven management experience in social services or related fields, with a focus on developmental disabilities or behavioral health Demonstrated program development skills with a track record of successful implementation and evaluation Extensive working knowledge of working with people with developmental disabilities and understanding their unique needs Strong relationship management abilities to foster partnerships within the community and among stakeholders Extensive knowledge of MN 245D standards.
