Community Inclusion Program Manager Integrity Supports & Services (ISS) is looking for a positive, confident, high-energy leader to manage our Community Inclusion Program serving adults with developmental disabilities. What You'll Do Lead, coach, supervise, and motivate Direct Support Professionals Create a positive, fun, and accountable work environment Be actively involved with participants and staff throughout the day Ensure staff are consistently engaging with participants—not simply supervising them Set clear expectations and address performance concerns when expectations aren't being met Develop creative, meaningful activities at our center Find new community experiences, volunteer opportunities, events, activities, and partnerships Help ensure activities reflect participants' interests, goals, abilities, and preferences Coordinate staffing, schedules, outings, transportation, and daily program operations Monitor quality of services, documentation, staff performance, safety, and compliance Work closely with ISS leadership to identify problems and implement solutions Bring new ideas to the table and continuously look for ways to make the program better Participant Case Management This position also includes case management and oversight of assigned participants . The Manager is expected to know the individuals we serve—not simply manage the employees supporting them.
Responsibilities include:
Maintain a working knowledge of each assigned participant's ISP, goals, preferences, behavioral supports, safety plans, medical protocols, supervision needs, rights, and other participant-specific requirements Monitor participant progress toward goals and ensure staff are actively working on identified outcomes Review participant documentation for quality, accuracy, completeness, and follow-through Ensure staff understand participant-specific plans and receive necessary training Identify changes or concerns involving a participant's health, behavior, needs, services, or quality of life and ensure appropriate follow-up Communicate professionally with Service Coordinators, guardians, families, Shared Living Providers, residential providers, and other members of the participant's team Participate in ISP and team meetings as assigned and follow through on action items Assist with new participant admissions, transitions, service changes, and discharges Ensure participant activities and community experiences are meaningful, person-centered, and connected to individual goals and interests Advocate for participant choice, independence, dignity, rights, and meaningful community inclusion Case management at ISS means taking ownership. The Manager should know what each participant needs, what staff should be working on, whether those supports are actually being provided, and when something needs to change. Preferred Experience Leadership, supervisory, or management experience is strongly preferred. Experience in developmental disabilities, human services, behavioral health, healthcare, education, community programming, or another people-centered environment is highly preferred. Most importantly, we are looking for the right leader. Someone who can build a strong team, create a culture people enjoy being part of, and still maintain high expectations.
