Develop and manage a diversified portfolio of referral sources and strategic community partnerships to achieve admission and growth targets across assigned territories. Cultivate relationships with hospitals, behavioral health providers, criminal justice agencies, housing organizations, and other key stakeholders to generate referrals and expand Outreach's network. Serve as a subject matter expert on Outreach's continuum of care, coordinating with internal program and intake teams to streamline referral workflows and connect individuals to appropriate services. Create and execute territory and community action plans by profiling accounts, prioritizing opportunities, monitoring performance metrics, and adjusting strategies to maximize growth. Deliver presentations, negotiate partnerships, resolve stakeholder concerns, and provide ongoing follow-up to ensure exceptional partner satisfaction and long-term collaboration. Represent Outreach as a community ambassador through networking, events, and regular travel while advising leadership on market opportunities, strategic partnerships, and business development initiatives.We require:
High School Diploma/GED Equivalent; Bachelor's Degree a plus Qualified NYS Driver's License and Car Preferred and/or access to public transportation 3-5 years of experience in community outreach and/or sales. Knowledge and experience working with at-risk populations a plus Bilingual English/Spanish preferredOutreach:
