Position Overview:
Beautiful Minds Group is seeking an experienced Case Manager to oversee, coordinate, and advocate for the individualized care and service plans for individuals with developmental disabilities. The successful candidate will have a strong background working within the DD field and must align with the guidelines set forth by the Ohio Department of Developmental Disabilities (DODD).
Key Responsibilities:
Oversee daily operations between site management and staff to ensure quality care of assigned clients Coordinate and facilitate meetings, including ISP meetings, with individuals, families, guardians, SSA representatives, and other care team members. Serve as a primary advocate for assigned individuals, ensuring their needs, preferences, and rights are respected and upheld. Monitor service delivery, ensuring quality and compliance with all applicable Ohio DODD rules and regulations for multiple assigned sites. Maintain accurate documentation and records as required by Beautiful Minds Group policies and Ohio DODD standards. Team work mentality, exceptional drive and accountability Provide crisis intervention and problem-solving support as needed. Participate in ongoing professional training and development to remain informed of best practices and regulatory requirements.
Required Qualifications:
Minimum of 3-5 years of direct experience working within the DD field as a Lead DSP, Site/Residential Manager Thorough understanding of Ohio Department of Developmental Disabilities (DODD) guidelines and regulations. Experience developing, implementing, and monitoring Individual Service Plans (ISPs) for multiple clients and sites. Exceptional organizational, interpersonal, and communication skills. Valid Ohio driver's license, reliable transportation, and current auto insurance. Ability to pass a criminal background check per Ohio DODD standards. Possess valid and up to date relevant certifications such as CPR/First Aid and 8hr DSP Certification and Medication Administration recognized by the
Ohio DODD High School Diploma or Equivalent Preferred Qualifications:
