The Care Management Supervisor responsible for the direct supervision of several individual service programs offered by the Agency, and coordinating care management staff to vend these services in the most efficient and effective manner possible. The Care Management Supervisor shall possess 3 years of experience in public or private social work and a Bachelor Degree or any equivalent combination of experience and training including successful completion of 12 semester hours of college level courses in sociology, social welfare, psychology, gerontology, or other related social sciences.
