Job Overview The Manager Social Care supports the implementation of the Office of Health Impact's Social Care strategy within AmeriHealth Caritas North Carolina. This role serves as a hands-on, working leader who directly engages with community organizations while overseeing the day-to-day operations and performance of Social Care Partners. As the Manager Social Care, you will develop and sustain strategic community partnerships; implement standardized social care programs, processes, and workflows; and work closely with internal teams and external stakeholders to improve member outcomes. You will maintain consistent execution of enterprise-wide Social Care initiatives while thoughtfully adapting local implementation to reflect the needs, resources, and priorities of the communities served. Work Arrangement This is a remote role requiring residency in one of the following counties in
North Carolina:
Onslow, Jones, Pitt, Duplin, Wayne, Lenoire, or Greene Monday through Friday, 8:30 AM EST to 5:00 PM EST Up to 30% local travel to meet with social partners and stakeholders within the community Limited overnight travel to attend conferences Education and Experience Bachelor's Degree in Public Health, Social Work, Healthcare Administration or related field required Demonstrated community and stakeholder engagement experience required; previous non-profit experience preferred Minimum of 3 years of experience directly leading teams within a matrix environment Experience within a health plan or managed care organization focused on community outreach preferred Licensure Must possess valid driver's license and ability to travel within the designated service area Skills and Abilities Cultivates strategic partnerships and fosters cross-functional collaboration Data-driven, evidence based approach to identify targeted preventions and improve programming quality and outcomes Strong working knowledge of MS Office to include SharePoint, Teams, Outlook, Word, PowerPoint, and Excel Our Comprehensive Benefits Package Flexible work solutions including remote options, hybrid work schedules, Competitive pay, Paid time off including holidays and volunteer events, Health insurance coverage for you and your dependents on Day 1, 401(k) Tuition reimbursement and more.
