Job Overview The PAS Community Outreach & Program Supervisor is primarily responsible for developing referral relationships, increasing appropriate PAS referrals, supporting census growth, and representing Sunglo Personal Assistance Services throughout the community. The position also serves as a qualified PAS Supervisor when designated by the Agency and is responsible for selected client supervisory, attendant oversight, service-plan, and regulatory functions in accordance with applicable Agency policy and Texas requirements. The position is expected to spend the majority of working time in community outreach, referral development, relationship management, referral follow-up, and census-development activities, while maintaining sufficient involvement in PAS operations to understand program needs and fulfill assigned supervisory responsibilities. Responsibilities
PRIMARY DUTIES - COMMUNITY OUTREACH & PROGRAM GROWTH
Develop and maintain referral relationships with physicians, clinics, hospitals, rehabilitation facilities, nursing facilities, Medicaid managed care organizations, community agencies, social workers, discharge planners, schools, senior/community organizations, and other appropriate referral sources. Educate referral sources regarding Sunglo PAS services, payer participation, service areas, eligibility pathways, private-pay options, and referral procedures. Generate and follow up on appropriate PAS referrals. Maintain consistent contact with established referral sources and develop new sources throughout the assigned service area. Track referral activity, pending referrals, admissions, lost referrals, and barriers to conversion. Work closely with Intake and PAS staff to ensure referrals are followed promptly and missing information is obtained. Identify referral sources with the greatest potential for sustainable PAS census growth. Conduct scheduled field visits, presentations, community events, networking activities, and referral-source meetings. Maintain a weekly outreach plan and document completed visits, contacts, referrals, and follow-up activity. Provide Administration with regular updates regarding referral trends, competitive activity, payer opportunities, staffing barriers, and growth opportunities. Promote all appropriate Sunglo PAS service options without guaranteeing eligibility, authorization, payer coverage, hours, staffing, or admission. Support ethical patient choice and avoid any activity that could be interpreted as offering compensation, gifts, employment benefits, or other value in exchange for referrals or client selection. Coordinate with Sunglo's other programs when an individual may benefit from another service within the care continuum, consistent with patient choice and applicable requirements.
SECONDARY DUTIES - PAS PROGRAM SUPERVISION
When designated as the PAS Supervisor and qualified under applicable requirements, the employee will: Conduct required client assessments, supervisory visits, reassessments, and service-plan activities assigned by the Agency. Review client needs and ensure services are consistent with authorized tasks and the service plan. Monitor attendant performance and obtain client/family feedback regarding service delivery. Identify service, staffing, competency, EVV, documentation, or client-safety concerns and escalate them appropriately. Participate in orientation, competency review, coaching, and performance monitoring of PAS attendants. Ensure changes in client condition, hospitalization, complaints, abuse/neglect/exploitation concerns, service interruptions, and other significant events are reported according to Agency policy. Coordinate with an RN whenever health-related tasks require nursing assessment, delegation, or supervision. Assist with continuity-of-care issues and urgent staffing concerns when needed. Participate in PAS quality-improvement and compliance activities. Qualifications When designated to perform PAS supervisory functions, the employee must meet the qualifications applicable to a PAS supervisor under 26 TAC §558.404(g) through one of the permitted education/licensure/supervisory-experience pathways. The Agency should verify and retain supporting documentation before assignment of those duties. Current PAS rules remain under §558.404; HHSC has proposed amendments to that section, so the Agency should continue monitoring the final adopted version. Experience Prior healthcare marketing, community outreach, PAS/PHC, home care, Medicaid services, business development, or referral-development experience preferred. Demonstrated ability to build referral relationships and generate measurable census growth. Experience working with MCOs, physician offices, hospitals, SNFs, community organizations, or other healthcare referral sources preferred. PAS or attendant-services supervisory experience preferred. Bilingual English/Spanish preferred.
