Job Description

Overview Copley Hospital, located in Morrisville/Stowe VT, is looking for a Community Health Integration Manager to join the team! The Community Health Integration manager is a member of the Quality Department team tasked with supporting the development and implementation of organizational strategies to address population health goals through management of the Blueprint and Community Health Team/Initiatives. This position is Full-Time (PT available). Copley Hospital has a variety of shifts available across Full-Time, Part-Time, and Per Diem needs. Compensation:
$93,600-99,840/year. The posted salary range reflects compensation for candidates with limited experience. An offer may vary based on factors such as experience, education, skills, internal equity, and market data. An offer of employment may be outside of the posted range based on those factors.
Responsibilities Essential Functions:
1. Manage Community Health Team to implement population health strategies through integrated workflows. a. Implement structures to monitor status of team objectives i.e. team huddles, workplans. b. Managers Meetings, 1:1 check-in meeting with team members, and conduct reviews. 2. Develop and implement strategies to address needs on the Community Health Needs Assessment and collaboration with community stakeholder's initiatives. 3. Manage community partnerships for Copley to increase collaboration between primary care and community-based organizations. a. Champion community collaborative initiatives. b. Communicate about Community Health Team and population health programs to the community and partners. c. Establish referral protocols with community partners. 4. Manage Blueprint projects in collaboration with Blueprint Facilitator. Attend Blueprint meetings as needed. a. Pregnancy Intention Initiative b. Self-Management Programs c. Community Health Team Support d. MOUD program support 5. Monitor/Manage Community Health Team budget a. Blueprint, grants, and other funding. b. Tracking expenses (training, travel, etc.). 6. Co-develop strategies in alignment with clinical and quality initiatives to leverage ACO and other payor partnerships to support enhancement of care coordination and improve health outcomes. a. Leverage care coordination to meet quality measure goals for specific cohorts b. Support Copley Emergency Department (ED) utilization, inpatient admissions, hospital readmissions initiatives 7. Manage new projects and develop implementation strategy for Community Health Team and Blueprint programs as needed
Qualifications Knowledge and Experience:
1. Required skills, knowledge and abilities: a. Project management b. Team management c. Strategic planning d. Meeting facilitation e. Budget management f. Excellent communication and collaboration skills g. Proven critical thinking and problem-solving skills 2. Registered Nurse preferred 3. Bachelor's degree in related field. 4. Five years or more of experience managing projects, teams, and strategic development. Experience working with community partners and leading collaborative work.
