What You'll Do:

As Director of the Encore Program, you will serve as the program's leader, educator, and ambassador. You will develop and standardize dementia-focused care practices, educate interdisciplinary hospice teams, provide guidance and education to families, and help integrate meaningful sensory and non-pharmacological interventions into patient care. You will also collaborate with Clinical Directors and Patient Relations Managers to introduce the Encore Program to memory care communities, assisted living facilities, skilled nursing facilities, and other healthcare partners throughout Pennsylvania. This position allows you to combine clinical knowledge, education, leadership, creativity, and community outreach in a role where your work can directly improve the experience of patients and families facing advanced dementia.

What We're Looking For:

Certified Dementia Practitioner (CDP) certification or equivalent dementia-focused training 3-5+ years of experience working with advanced dementia or memory care populations Experience with education, training, or program development Strong leadership, communication, and presentation skills A compassionate and patient-centered approach A passion for educating others and improving dementia care Make an Impact That Matters At Serenity Hospice, our mission is to provide exceptional comfort care every day until the end.
