Job Description

We are seeking an experienced, detail-oriented, and compassionate Program Manager for Supportive In-Home Services to lead our community -based support team. The ideal candidate will oversee daily operations, supervise direct support staff, and ensure person-centered care that complies with state licensing standards, Medicaid Waiver regulations, and agency policies. A primary core competency for this role is expertise in clinical and regulatory documentation, specifically the drafting implementation, and evaluation of a Part V Individual Support Plan (ISP), goal tracking, and Quarterly Reviews (Quarterlies)
Education:
Bachelor's, degree in Social Work, Human Services, Psychology, Special Education, or related field (or equivalent combination of education and leadership experience in HCBS/in home services).
Work Location:
Hybrid remote in Chesterfield, VA 23832
