Job Description:
We Empower Home Care is seeking a dedicated and compassionate Client Care Coordinator to join our team in Indianapolis. As a Client Care Coordinator, you will play a crucial role in managing caregiver schedules, client care plans, and ensuring the highest quality of care is delivered to our clients. This position requires a proactive approach to maintaining client records, supporting our caregiving staff, and ensuring compliance with all legal and company regulations.
Responsibilities:
Care Coordination:
Manage and coordinate the schedules of caregivers and the care plans for clients to ensure seamless service delivery.
Interviewing:
Finding the best caregivers for the clients.
Training and Support:
Ensure that all caregivers receive the necessary training and ongoing support to meet the needs of our clients effectively.
Record Keeping:
Maintain accurate and up-to-date client records, including care plans, service logs, and health records, ensuring privacy and confidentiality.
Quality Oversight:
Oversee the quality of care provided to clients, conducting regular reviews and updates of care plans as needed.
Regulatory Compliance:
Ensure compliance with all relevant laws and regulations as per 455 IAC 2-15-2(b)(6), maintaining high standards of care and operational integrity.
Family and Client Support:
Act as a liaison between caregivers, clients, and families, providing them with necessary information, guidance, and support.
Supervision:
Supervise caregiver performance, ensuring adherence to established care plans and protocols.
Administrative Duties:
Handle administrative tasks related to client care, including documentation and compliance checks.
Additional Duties:
Coordination of Care Services:
Coordinate non-medical care services for clients, ensuring their daily needs are met in a dignified and respectful manner.
Performance Supervision:
Monitor and evaluate caregiver performance to ensure compliance with care plans and quality standards.
Compliance and Safety:
Ensure that all staff and clients adhere to established health and safety protocols and regulations.
Qualifications:
Must have experience in interviewing. Prefer experience in care coordination or a similar role in home care or healthcare settings. Excellent organizational and communication skills. Strong leadership abilities and a team-oriented approach. Knowledge of healthcare regulations and standards. Compassionate and patient-centered approach to care. Must bring your own laptop at the beginning.
Shift:
