We are seeking a reliable, organized, and compassionate Care Coordinator/Scheduler to join our healthcare team in Burnsville, MN. The Care Coordinator/Scheduler plays an important role in coordinating client care, managing caregiver schedules, supporting caregiver recruitment and onboarding, and helping ensure clients receive consistent, quality care. This position requires strong communication, attention to detail, problem-solving skills, and the ability to work effectively in a fast-paced environment. Key Responsibilities Coordinate client and caregiver schedules to meet client care needs. Match caregivers with clients based on client needs, caregiver qualifications, and availability. Coordinate coverage for open shifts, call-offs, and schedule changes. Communicate with clients, families, caregivers, case managers, and county staff. Assist with caregiver recruitment, hiring, onboarding, and training. Respond to phone calls, emails, and daily scheduling requests in a timely and professional manner. Maintain accurate client, caregiver, scheduling, and required compliance documentation. Monitor schedules and help identify and resolve staffing or coverage issues. Participate in staffing meetings and team meetings. Provide on-call support as needed. Maintain confidentiality and follow company policies and applicable healthcare requirements. Perform other duties related to care coordination and scheduling as assigned. Qualifications At least 1 year of customer service experience . Healthcare or home care experience preferred. Bilingual Hmong and English preferred/required. Strong verbal and written communication skills. Excellent organizational and time-management skills. Ability to multitask, prioritize, and work effectively in a fast-paced environment. Strong attention to detail and problem-solving abilities. Basic computer skills, including Microsoft Word and Excel. Ability to work independently while also being an effective team member. Reliable, responsive, professional, and compassionate. Ability to handle confidential information appropriately. Pay:
$22.00 - $24.00 per hour Expected hours: 40.0 per week
