Job Overview Provide a full range of administrative duties and independently administer several assigned responsibilities, ensuring exceptional customer service for Home Care services. This position will also encompass the duties of the CFSS Supervising Professional (SP), providing supervision, support, training and evaluations of activities of Workers for Clients receiving services. The SP must comply with service deliver plans in accordance with federal and state regulations and the agency's policies and procedures to ensure the quality delivery of services to all Clients served. JOB RESPONSIBILITIES
Provide training and development to Workers enabling them to perform specific tasks for Clients receiving services. Provide development of the Worker(s) that includes: The review of a Client's service delivery plan Observation, supervision, and evaluation of the Worker Evaluation of service outcomes with Client/Representative Evaluation of site and whether outcomes are being met Modification of Client's service delivery plan and re-training Workers as needed Complete the service delivery plan with the Client/Representative following the initial development by a Consulting agency with the Client/Representative. Complete and update each Client clinical record. Enter into a written agreement with G & H of Greater Minnesota Inc. and the Client/Representative.
SP SPECIFIED POSITION REQUIREMENTS
The SP will be responsible for specified documentation and on-site training and development job duties. Documentation Requirements - the SP is responsible for documenting all details of training and development, including: Date and time of visits. Number of hours spent on training and development. Change in the Client's condition requiring additional training. Specific actions the Worker is performing to address the Client's condition change. Client's/Representative's comments and/or concerns. Joint Worker development by the SP with the Client/Representative. Communication Requirements - the SP must ensure the Client/Representative understands their rights by taking the following actions within five (5) working days of the start of services and annually: CFSS Rights and Responsibilities (DHS-6893R) or an alternative form when the Client needs it to understand their rights.
POSITION DESCRIPTION
Document that the Client/Representative received a copy of their rights. Enter into a written agreement with the Client/Representative that describes the roles and responsibilities of each party. Ensure the Client/Representative participates in and approves of the evaluation of their services on a regular basis. Document the Client preferences for Workers. Provide the Client/Representative with information about: Ø Agency's policies and procedures Ø How much the CFSS services cost, and under what circumstances the Client might be responsible for any costs. Ø Any limitations on the services the agency provides. Ø How to file a complaint Ø Agency's ability to meet the needs identified in the Client's service plan delivery. Ø Staffing strategy, including proposed frequency and scheduling process. Ø Contact information for the Ombudsman Office for Long-Term Care. Review of Services Requirements - the SP must conduct reviews: Within 90 days of beginning services (in person) At least quarterly for the first year of services (in person) At least quarterly for the second year and the duration of services (in person once per year and may conduct the remainder remotely) Within 30 days of learning about a Client's change in condition

Service and Worker reviews may be combined. Exceptions to Review of Services and Worker Requirements - The SP may conduct the required reviews remotely for a Client with chronic health conditions or a severely compromised immune system when: The Client requests a determination from their primary health care provider. The primary health care provider determines remote supervision is appropriate. The primary health care provider documents their determination of need for remote supervision. Copies of the primary health care providers documents are retained in the Client's service delivery plan with the CFSS agency provider.

The initial review of services and first evaluation of a Worker's competency must be conducted in-person.

The Client may request to return to in-person reviews at any time. 5. Documentation of Review Information - At each review, the SP must document the following: The service delivery plan correctly states the Client's service needs. The services are supporting the Client to accomplish goals identified in the service delivery plan. The Worker(s) are competency to provide services identified in the service delivery plan. Any concerns of the provider agency or Client/Representative (if applicable). Any action that must be taken as a result of the review, such as updating the service delivery plan or providing additional Worker training. 6. Worker Training and Development Budget - The agency may bill for SP time used to: Work with the Client to finalize items within the agency's scope in the service delivery plan. Supervise and evaluate services. Train, develop, evaluate, and supervise Workers Make updates to items within the agency's scope in the Client's service delivery plan, as needed.
ADMINISTRATIVE JOB RESPONSIBILITIES
Potential Client Customer Service:

Client Intake Approval:
- Confirm appropriate company licensing to provide service

- Research and confirm sufficient staffing availability
Client Files:
Create new client file(s) in Electronic Visit Verification (EVV) system or paper when necessary Enter client assessment, authorizations, and contracts into EVV Review client files in EVV to verify completed information from client visits Set client visit deadlines in EVV Notify appropriate office staff of new and updated information in
EVV Intake Scheduling:
Coordinate schedule(s) of new client intake(s) with nursing staff Coordinate schedule(s) of new client intake(s) with leveled staff when required Coordinate schedule(s) of new client intake(s) with the 245
D Designated Coordinator Client Assessments:
Contact appropriate agency to receive current assessments for client files
Customer Service:
Provide customer service and issue resolution to clients
Data :
Monitor data reports for: 60-day notices. Maintain client discharge log, client roster, assessment dates, client file data, hospitalization/death notification reports, incident reports, hospitalization/death notification, and vulnerable adult/child reports and client information
Notices:
Provide 60-day reassessment notices to County Health and Human Services for MN DHS clients Communicate with CMDE to obtain client renewal dates
Service Agreements:
Print and review all service agreements to confirm accurate information Provide copy of service agreements to office/support staff as necessary
New Employee Processing:
Coordinate with the Employee Relations Coordinator to request staff for openings with existing clients and potential clients. Coordinate with the Employee Relations Coordinator to determine specific staff that best compliment clients and their needs.
Scheduling:
Coordinate and direct employees for schedule changes, in conjunction with the Employee Relations Coordinator, as necessary. Daily EVV approval/denial Timecard and Documentation Approval Review and approve/deny nursing staff timecards Review and approve/deny nursing staff's client documents
On-Call:
Provide on-call assistance in emergency after-hours situations
Attend Weekly Administrative Meetings Quality Improvement:
Participate in Quality Improvement Team meetings Provide information/documentation for quality improvement Other Duties as Assigned Experience Proven experience in operations management or coordination within a professional setting. Strong organizational skills with the ability to manage multiple priorities simultaneously. Excellent communication skills for effective collaboration across departments and teams. Demonstrated leadership capabilities with experience supervising or mentoring staff. Proficiency in using operational software tools and data analysis platforms. Ability to adapt quickly to changing priorities while maintaining attention to detail. Experience in process improvement initiatives is a plus.
