GED Full Job Description Overview:
Under the direction of the Staffing Manager, coordinates scheduling of all Home Care Aides/Geriatric Caregivers and Home Care LPNs designated to care for PACE participants to ensure adequate and safe staffing within the confines of established staffing patterns and the Union contract. Responsible for accurate record keeping regarding hours worked, call-ins, absences, etc. and bringing concerns to the attention of the Staffing Manager and Home Care Manager.Responsibilities:
Job duties specific to this position: Schedules Geriatric Care Givers and Home Care LPNs to meet the needs of PACE participants as determined by the care plan/orders. Daily evaluates and adjusts staffing to ensure appropriate participant care as outlined in each individual's care plan/orders. Works to cover all call-offs of Geriatric Care Givers and Home Care LPNs in a timely manner and reports issues to Staffing Manager. Carefully processes all changes in services for participants as ordered by the PACE Team, and ensures aide and nursing visits are scheduled accordingly. Communicates accurately with nurses, Geriatric Care Givers/Contract Agencies, Home Care Managers and PACE administration to cover open shifts. Answers phone promptly and exhibits telephone courtesy for a high volume of calls. Greets callers positively and with proper identification. Tracks all calls coming in to the department via call log. Communicates professionally and appropriately with all contacts. Communicates extensively with aides, participants and their families, Staffing Manager, Director of Patient Services, Day Center Supervisor, Home Care Manager, and the Home Care Agency to achieve appropriate staffing. Reports problems/issues i.e., union grievances, staff complaints/issues, interdepartmental staffing issues, participant/family complaints/issues to appropriate person and assists in resolution as appropriate. Effectively uses and assists in development and improvement of computerized staffing program. Prepares four-week schedules for Geriatric Care Givers and Home Care LPNs. Prints and mails participant schedules as determined by living situation weekly. Prints aide schedules to all shared aide buildings weekly. Print staffing level report twice a day and as needed to all shared aide buildings and day centers. Exercises sound judgment based upon acquired knowledge, participant need and program service scope. Has access to participant records and reports where maintaining confidentiality is essential. Must possess ability to self-direct daily routines in support of program objectives; work cooperatively with others, communicate effectively, and possess physical and emotional ability to perform work assigned. Must demonstrate flexibility, creativity, and responsiveness in meeting individual and program goals. Cooperates with leadership staff in implementing planned changes and activities to improve participant care or overall functioning of the PACE program as it relates to staffing. Responsible for communication of updates, schedules, and policies and procedure affecting aides and nurses via written, verbal, or electronic communication. Adapts to changes in job responsibilities in a professional manner. Participates in problem solving and makes recommendations and suggestions for positive procedure changes. Adheres to rules in the union contract regarding hours worked by staff, overtime, mandation processes and all staffing tasks Responds to and handles situations appropriately, following the appropriate chain of command. Recognizes and respond to emergent situations efficiently and effectively by managing self, participants, staff, and other appropriately. Works closely with Home Care Managers to schedule new employees independently only after receiving notification that appropriate paperwork and competencies have been completed. Maintains management of information to include staff and participant confidentiality issues. Maintain patience and a cooperative attitude regardless of the situation presented. Attends staff and in-service meetings as required; participates in QI task forces and activities as requested. Practices and teaches safety awareness. Identifies and reports or corrects any possible safety or environmental issues. Understands all HIPAA and other compliance & regulatory requirements associated with their role, including completion of required internal or external training/certifications. Performs a variety of related duties as assigned.Qualifications:
High School diploma or GED required ASA degree or LPN license preferred. 2 years experience with scheduling staff preferred. Excellent communication skills. Ability to communicate clearly in person, in writing, or by computer. Excellent organization skills. Proficient in MS Office applications and able to learn new software rapidly. Ability to use standard office set-up including computers, copy machines, printers, etc.PHYSICAL REQUIREMENTS
