Position Summary Diamonds & Pearls Health Services is seeking an experienced, dependable, and highly organized Care Coordination & Scheduling Specialist (Dual Role) to support both our Home Health and DODD Waiver divisions. This position plays a vital role in ensuring participants receive uninterrupted, quality services by coordinating staff schedules, maintaining communication with interdisciplinary team members, and providing direct care coverage when needed. This is a dual-role position that combines office operations with field support. The Care Coordination & Scheduling Specialist must be flexible, solution-oriented, and comfortable working in a fast-paced environment while maintaining exceptional customer service. Essential Duties and Responsibilities Coordinate daily, weekly, and long-term schedules for Home Health and DODD Waiver services. Fill open shifts due to call-offs, vacations, emergencies, and staffing shortages. Communicate scheduling updates to employees, supervisors, participants, and families. Send timely communication notes and participant updates to Case Managers, Service and Support Administrators (SSAs), and other interdisciplinary team members as appropriate. Monitor authorizations and approved service hours to ensure staffing aligns with participant plans. Verify employee credentials, required training, and compliance before assigning shifts. Assist with orientation scheduling and new employee assignments. Monitor overtime and staffing utilization. Provide on-call support on a rotating basis. Work collaboratively with Operations, Nursing, Compliance, House Managers, Human Resources, and Scheduling. Maintain accurate documentation within the agency's scheduling and EMR systems. Provide office support including answering phones, assisting employees, and responding to participant concerns. Float into the field to provide direct care services when staffing shortages or emergencies occur. Participate in after-hours staffing support as assigned. Perform other duties as assigned. Minimum Qualifications High School Diploma or GED required. 75-hour Home Health Aide (HHA) Certificate required; STNA or CNA accepted. DODD Direct Support Professional experience preferred. Minimum of one year of healthcare scheduling experience required. Home Health and/or DODD scheduling experience strongly preferred. Experience using EMR and scheduling software (Alora experience preferred). Proficiency in Microsoft Office 365, including Outlook, Excel, and Word. Typing speed of at least 35 words per minute. CPR and First Aid Certification (or ability to obtain upon hire). Valid Ohio driver's license, reliable transportation, and automobile insurance. Ability to pass all required background checks and maintain agency employment requirements. Knowledge, Skills, and Abilities Knowledge of Home Health and DODD Waiver regulations. Strong organizational and multitasking skills. Excellent verbal and written communication. Outstanding customer service and conflict-resolution abilities. Ability to prioritize multiple tasks in a fast-paced environment. Strong attention to detail and problem-solving skills. Ability to maintain confidentiality in accordance with HIPAA requirements. Ability to work independently while functioning as part of a collaborative team. Physical Requirements Ability to sit, stand, bend, walk, and lift up to 50 pounds. Ability to assist participants with activities of daily living when working in the field. Ability to travel between participant homes, agency offices, hospitals, and community locations. Schedule Full-Time Monday-Friday (office hours may vary) Rotating on-call responsibilities Evening, weekend, and holiday availability as needed Flexibility to respond to staffing emergencies Preferred Qualifications Home Health Aide (HHA) Certification. Experience scheduling both Home Health and DODD Waiver services. Previous supervisory or staffing coordination experience. Experience with Alora or comparable EMR/scheduling software. $19.00 - $25.00 per hour
