In Office Position Summary Summit Home Health & Hospice is seeking a highly organized and detail-oriented Scheduler to coordinate staffing, patient visit scheduling, provider communication, and administrative workflow support. The Scheduler serves as a central point of communication between clinical staff, providers, facilities, and office personnel to ensure timely patient care and efficient daily operations. Essential Duties and ResponsibilitiesScheduling & Staffing Coordinate and maintain clinician schedules within our charting system Process and manage staffing requests. Monitor visit schedules to ensure patient care needs are met. Assist with staffing adjustments due to schedule changes, call-offs, and urgent patient needs. Coordinate respite scheduling as appropriate. Missed Visit Management Run missed visit reports. Communicate missed visits to the appropriate provider or supervisor. Track and follow up on unresolved missed visits. Maintain accurate documentation of scheduling changes and visit completion. Patient Care Coordination Monitor open benefit periods and assist with associated scheduling needs. Coordinate communication between clinicians, facilities, providers, and office staff. Ensure required documentation is routed to the correct parties in a timely manner. Support continuity of care by maintaining accurate scheduling records. Documentation & Facility Communication Send Plans of Care (POC), IDG, and IDT documentation to facilities as required. Track and document completed transmissions. Maintain organized scheduling and communication records. Communication Management Manage and monitor company chats. Export company communications when required. Serve as a communication resource for clinicians and administrative staff. Coordinate phone communications related to scheduling and patient care activities. Administrative Support Answer and route incoming calls appropriately. Assist with office operations as assigned. Maintain confidentiality of patient, employee, and company information. Support special projects and operational initiatives as requested. QualificationsRequired High school diploma or equivalent. Strong organizational and time-management skills. Excellent verbal and written communication skills. Proficiency with Microsoft Office (Outlook, Word, Excel, Teams). Ability to manage multiple tasks while maintaining attention to detail. Ability to work independently and as part of a team. Preferred Previous healthcare scheduling experience. Home Health or Hospice experience. Experience with Kinnser/WellSky or similar EMR systems. Knowledge of Medicare and healthcare documentation processes. Pay:
$18.00 - $22.00 per hour Expected hours: 40.0 per week
