Our Certified Apartment Program has 2 openings for a Service Planner. This is a full time position and the hours are Monday - Friday 8AM-4:30PM (hours are somewhat flexible). Minimum qualifications for

Service Planner:

High School diploma or equivalent 1 year experience in the Human Services field Valid NYS driver's license required Ability to type, and to write clearly and objectively Service Planner responsibilities include: Collaboration with staff and clients on developing service plans according to OMH and Medicaid regulations Review of weekly contact notes Perform quarterly chart audits Completion of documentation according to agency standards When down staffed, coverage of clients is required.
