Professionals Job Summary AgeWell PACE is seeking a dedicated and compassionate bi-lingual (Spanish & English) team member to join our PACE Day Center. This part-time, non-benefited position plays a vital role in supporting the well-being and social engagement of our participants—frail older adults aged 55 and up—who wish to remain independent in their own homes. Key responsibilities include assisting with patient care activities, serving lunch, and performing other tasks as needed. This role is essential to fostering a supportive and inclusive environment for our participants.

Position Responsibilities Communicates effectively with participants, families, staff, and other departments and agencies. Works effectively in a team setting Provides basic personal care assistance and assistance with any other activities of daily living across settings to assigned participants. Aid with nursing care, restorative therapies, activities and meals across care settings. Documents treatment/procedures, observations, nursing care and patient activities on appropriate forms. Participates in teaching and learning process with others. Follow guest relations policies and procedures. Abides by all established policies, rules and regulations. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to supervisor. Possess the Ability To Maintain a positive, flexible, and solution-focused attitude, with both the ability to collaborate effectively in a team setting and to work independently. Provide feedback to support IDT decision making. Be organized within busy center and clinic environments. Maintain effective interpersonal relationships cross-departmentally, and, as applicable, with agencies, members, providers, and the public. Communicate effectively, both verbally and in writing, with individuals from varying cultural backgrounds. Prepare concise and clear reports. Multi-task on projects and their respective activities, timelines, and issues. Effectively utilize computer and software tools. Only act within the scope of their authority to practice. Work in a dynamic and fast-changing environment. Work within an effective team environment, where each team member is held accountable for their independent work. Approach the role with an openness to new ideas, cultural humility, and acknowledgment of the dynamics of privilege. Pass a criminal background which will verify that the candidate is: not excluded from participation in Medicare and Medicaid programs; has not been convicted of criminal offenses related to their involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act; and have not been convicted of physical, sexual, drug or alcohol abuse. Regulatory policies, rules, and regulations. Familiarity and/or experience with geriatrics. Experience & Education High School degree or equivalent. Currently listed as NA1 with CA Division of Facility Service with no substantiated findings of neglect, abuse, or misappropriation of resident's property in a nursing home. Bilingual English/ Spanish required. Physical demands:
Employee must be able to sit for extended periods of time. Employee will also be actively present throughout the PACE center, engaged with employees, participants and with staff.
