One year of experience in an administrative support services role is required. Experience with computer databases and managing data is required. Experience with electronic health records, preferred. A degree in health or human services, preferred.What You Will Do:
The Care Coordination Associate (CCA) is responsible for supporting the Coordination of Community Services (CCS) team by coordinating and administering specific activities related to Coordination of Community Services for individuals served by the Development Disabilities Administration (DDA). This position provides clerical support and serves as the primary contact for the internal client record that is maintained in The Center's documentation system. This position requires a customer service perspective, to internal and external customers. This role also requires CCAs to adjust their workload and develop specific goals and plans to prioritize, organize and manage the multiple priorities of the CCS program. CCAs are both office-based and remote, however, CCAs must be able to provide a consistent daily presence for prompt response to client, family, SP, and provider needs/inquiries. This position reports to the Team Manager but works jointly as a member of the CCS team to maximize quality outcomes for individuals served by The Center. As a Care Coordination Associate, you will: Function as the primary contact for The Center's documentation system including opening/registering, closing and reassigning clients in the system. Provide initial setup and ongoing support for system access, including login troubleshooting and user assistance. Provide primary clerical support including word processing (memos, reports, letters, consents, correspondence) as well as a variety of other duties including photocopying, scanning, e-faxing, organizing/processing documents and filing. Conduct initial contact with clients/families and or care/support team to introduce the CCS team, verify demographic information, and explain program/services. Provide general information and handle general questions about the CCS program. Triage calls and other inquiries from clients/families, providers, and outside agencies; address the issue/concern or providing appropriate follow-up with assigned coworker to address issue/concern. Contact clients to collect or provide specific information as directed by the CCS. Document client activities in the external and internal documentation systems. Generate reports from both external and internal documentation systems. Locate and maintain information on available resources for CCS clients. Complete other duties as assigned by CCSs, Team Managers or Program Director (i.e., create forms, special projects/tasks, etc.). Process incoming mail and directing it to the appropriate team member. Serve as Administrator for councils/committees by performing administrative tasks including meeting minutes, schedule meetings and assist facilitator with other duties as needed. Manage the on-call schedule for Team Managers. Demonstrate the ability to respond to changing situations in a flexible manner in order to meet current needs, reprioritizing work as necessary. Organize job functions and work area to be able to effectively complete varied assignments within established guidelines.
