This is a full-time teleworking position with the expectation that you will be visiting clients in the community setting. Who We Are The Coordinating Center (The Center) has over 40 years of experience supporting children, youth, adults and seniors with complex medical needs and disabilities statewide. Specific qualifications are:
A master's degree with one year of experience as a CCS; or a bachelor's degree and three years' experience as a CCS required. Experience as a CCS Team Manager can be waived for education requirements. Demonstrate experience in social services intake and referral services, data collection, analysis, reporting, staff supervision, management or leadership, and monitoring the quality of services. Demonstrate competency-based skills and working knowledge in the following areas: Negotiation and conflict management, crisis management, community resources Determining the most integrated setting appropriate to meet the individual's needs Monitoring the provision of services to individuals Allied service delivery systems, including Medicaid, mental health, substance abuse, social services, juvenile justice, vocational rehabilitation, and corrections; and Regulations governing services for individuals with developmental disabilities. Proficiency in LTSS documentation system; computer skills including Word, Excel, and PowerPoint. Creative problem solving and flexibility and ability to manage complicated work processes and multi-task. Ability to communicate effectively with individuals with varied cognitive abilities and medical needs to establish professional relationships. Strong belief in the opportunities for people with disabilities to fully participate in the community and a willingness to work toward individuals' identified goals. Excellent communication skills-writing and speaking.
What You Will Do:
The Coordinating Center seeks a dynamic individual to join its Home and Community Coordination Services Division. The Coordination of Community Services (CCS) Team Manager supervises a team of CCS Coordinators responsible for targeted case management services for people with intellectual/developmental disabilities. The Team Manager carries a small caseload (no more than 10) and supports the mission of The Coordinating Center, by providing person- and family-centered service coordination, within the parameters of the CCS contract. Team Managers are responsible for monitoring compliance with specific contract requirements, maintaining data and providing technical and analytical support in contract planning, data extraction, report writing and maintenance of all documentation. This position works with The Center's CCS management team to maximize quality outcomes for individuals receiving CCS services. The CCS Team Manager reports to the CCS Director. While this is a teleworking position, there are hot spots and conference spaces available for individual coworkers and group meetings and trainings at The Coordinating Center, in Millersville, MD. Team Manager You Will:
Assume day-to-day coordination and oversight of the Coordination of Community Services (CCS) and provide direct supervision to team members. Monitor staff caseloads and ensures appropriate coverage. Ensure all contract requirements are met, including the philosophy of freedom of choice, placing no restrictions on the qualified participant's choice of providers and person-directed supports and services; and "core services" are delivered, including comprehensive assessment, development of the individual plan, referrals and related activities, and monitoring and follow-up. Interview, orient, train, and mentor newly hired staff on CCS concepts, service delivery and contract specific processes, tools, and procedures. Perform daily review of staff work to ensure billing compliance, providing feedback regarding the completeness, timeliness, and quality of documentation. Serve as a resource to staff for problem solving regarding client issues, contract requirements, policies and procedures, DDA eligibility; advise, support and take appropriate action. Continually assess the resources, learning opportunities, and training needed by staff. Communicate those needs to the Director for discussion and implementation of the course of action. Ensure timely completion of all performance evaluations and coaching and counseling sessions. Troubleshoot staffing issues with Director and takes appropriate action. Facilitate effective teamwork and maintain positive work environment. Maintain a small client caseload, to assist participants in selecting, obtaining, and monitoring the most responsive and appropriate services and supports. Lead by example and implements The Center's mission/values within the department. Conduct regularly scheduled team meetings. Participate in all mandatory training (i.e. person-directed supports, data collection/analysis, coaching, problem solving, conflict resolution, contract protocols and procedures) and workgroups and committees within the organization. Manage multiple priorities and tasks with flexibility to enhance CCS. Use data to drive decisions, plan, and implement changes as necessary, to improve services. Work collaboratively with internal/external partners to enhance needed communication and coordination of comprehensive care to client population. Ensure appropriate assessment, documentation and outcomes measurement in Care Management documentation system with The Coordinating Center and the State of Maryland's Long-Term Services and Supports (LTSS) system.
