Job Description

About Us We are a medical startup that is working to improve outcomes among senior patients in assisted living communities. We provide a hybrid in person and virtual clinic services and are looking for a team member to join the team who would be seeing their own panel of patients monthly in assisted livings and within patients' homes and supervised by a cardiologist and NP. Who We are Looking For We are looking for someone with experience in a clinical setting and a quick learner who is highly compassionate and passionate about caring for people. We are looking for team players with exceptional communication skills and experience in working with multidisciplinary teams. As a a care manager you will have support from the care manager supervisor who oversees patient care in addition to a nurse practitioner and cardiologist.
Duties:
Visit patients in their homes (2 days on road for patient encounters per week, 3 days virtual care management) Implementing the medical plan from provider visits. Coordinating appointments, assisting to order medications, and arranging follow up. Documenting care plans and gathering pertinent medical history Understanding patient needs and advocating for the patient Reconcile medications Monitor Vitals About 50% of time would be virtual, while 50% of time would be spent with patients in assisted living facilities, or in patient homes. Geographic range is within the Bellevue and Seattle area. Ideal candidate is someone who has keen interest in continuity of care and developing relationships with patients while also being efficient and organized. Qualifications Experience working with seniors and prior experience in the medical field 2+ years Clean driving record and background check Responsible and organized Benefits We offer a flexible schedule and the ability to be hybrid and remote 50% of the time. Mileage will be compensated at standard IRS rates after the first 10 miles of driving to a location.
Education/Licenses/Certifications:
Completion of an accredited program for Medical Assistants required Washington State Medical Assistant-Certified (MA-C) or Interim Certification for one year before obtaining MA-C Exceptions can be made for candidates with great potential at Enabled.
Minimum Requirements:
Three (3) years experience in physician's office or other medical facility preferred
