Job Description

The pay for this position is $44,850-$58,908.9150 annually. $500.00 sign on bonus Sometimes it takes only one person to make a difference in a life. As the Care Navigator Case Manager for children's services, you could be that person. The Care Navigator Case Manager leads service planning and care coordination navigation for children and families receiving mental health services, including managing referrals, providing primary care screening and monitoring and serving as a primary liaison with community partners.
