Case Manager

Overview We are seeking a dedicated and compassionate Case Manager to join our outpatient behavioral health team. In this vital role, you will coordinate patient care plans, facilitate communication among multidisciplinary teams, and ensure seamless transitions across levels of care. Your expertise will help optimize patient outcomes, promote efficient utilization of resources, and support clients through their recovery journey. Responsibilities Assess patient needs through comprehensive intake and evaluation processes, including medical history review and social determinants of health. Develop personalized care plans that align with clinical guidelines and patient preferences. Coordinate timely and appropriate discharge planning Collaborate closely with healthcare providers, social workers, and insurance companies to facilitate utilization review and management. Maintain accurate documentation within EHR systems while adhering to HIPAA regulations for patient privacy. Support patients with behavioral health needs providing linkage to community resources and other services as necessary ensuring they receive comprehensive support. REQUIRED. A

valid driver's license and active auto insurance. Familiarity with EMR/EHR systems and discharge planning processes. Excellent understanding of healthcare regulations including HIPAA compliance and medical terminology. Experience with managed care programs, utilization review, and case management processes. Ability to navigate complex behavioral health scenarios Exceptional communication skills combined with critical thinking to advocate effectively for clients' needs.
