Abraxas Youth & Family Services is seeking a dedicated Case Manager to support youth and families impacted by complex trauma . This position plays a vital role in helping youth successfully transition into family, school, and community settings through individualized case management, advocacy, therapeutic support, and professional collaboration with internal teams and community stakeholders. The ideal candidate is organized, trauma-informed, detail-oriented, and committed to creating positive outcomes for youth through intensive treatment planning and relationship-based care.Qualifications:
Bachelor's degree in social work or a related field, or equivalent work-related experience. At least six months of work-related experience with at-risk youth. At least 21 years of age. Criminal clearances (Act 33 and 34, specific state and child clearances, and, if necessary, FBI). Non-communicable diseases physical exam. Valid driver's license from the employee's state of residence. Valid registered vehicle insurance.Essential Functions :
Trauma-Informed & Therapeutic Care Provide case management services to youth experiencing the residual effects of complex trauma Utilize trauma-informed interventions and relationship-based approaches that support emotional regulation, growth, and healing Participate in intensive treatment planning for youth with high behavioral and emotional needs Facilitate therapeutic support to the youth Professionalism & Community Collaboration Maintain a high level of professionalism when working with courts, probation, external team, GALS, community providers, and family members Build strong partnerships with community stakeholders to ensure continuity of care and successful reintegration Represent youth needs professionally during court hearings, staffing meetings, Treatment plan meetings, and treatment reviews Attention to Detail & Documentation Complete reports, case notes, treatment updates, and required documentation accurately and on time Complete weekly check ins with caseload, document needs and support needs. Maintain close attention to deadlines, appointments, compliance standards, and youth progress tracking Ensure all communication and records meet licensing, contractual, and regulatory requirements Milieu Support & Overtime Expectations Provide direct support in the residential milieu environment when operationally needed Participate in overtime assignments to maintain safe staffing ratios and quality care for youth Assist with crisis response, supervision, and structured programming during staffing shortages or high-need periods Advocacy & Case Coordination Coordinate services with clinicians, families, external team, and outside agencies Assist youth in meeting court-ordered obligations such as restitution, community service, counseling, and educational goals Schedule visits, transportation, appointments, and transition servicesBenefits & Perks:
