Non-exemptResponsibilities:
The Residential Support Case Manager provides supportive case management services to tenants with mental illness in supportive housing and provides on-site visits to scattered site locations. The Case Manager will work with the tenant, treatment team, and the Mobile Residential Support Team (MRST) to identify issues and barriers that may prevent the individual from maintaining stability in the community. Through a mutually agreed goal plan, the MRST Case Manager will assist the tenant in seeking solutions to achieve mental health and health care goals, sobriety, housing stability and to prevent where possible, emergency room usage and inpatient hospitalization. Provide site visits, write progress notes collaboratively with the tenant (when possible), review service plan and goals. Help develop independent medication management skills. Provide linkages to community resources. Assist in securing vocational assessment services, job training and education programs. Provide advocacy services to gain appropriate entitlements and services. Intervene in situations requiring immediate attention to ensure safety of residents. Provide mediation, counseling, behavioral intervention and crisis management services. Consult and coordinate with clients' family and providers as needed for coordinated service delivery. Notify Program Supervisor of any medical or psychiatric emergency. Salary Range $42,900- $54,600 USD Incredible people doing meaningful work.
