At Hearts Aligned Inc., we stand beside families navigating the emotional and practical challenges of caring for a critically ill child. Our mission is to provide financial assistance, family support and advocacy to low-income families with a critically ill child. Through connection, community, and care, Hearts Aligned ensures no family faces hardship alone. Position Overview The Case Manager will be responsible for our family support programs covering Santa Barbara County . The Case Manager will work with the Executive and Program Director to ensure Hearts Aligned programs are being administered properly. The Case Manager will work with medical referral sources and ensure all families have completed all required forms; are assessed for eligibility; interviewed; and have a well-documented file to be accepted into one or more of Hearts Aligned programs. Key Responsibilities Build trusting relationships with families, social workers, healthcare providers and partnering organizations Assess and address the unique needs of each critically ill child and their family Ensure services are delivered with empathy and cultural sensitivity Understand preferred communication style of families

Maintain confidentiality and adhere to ethical standards, including HIPAA regulations Work and coordinate with each healthcare professionals Assist families in completing financial assistance applications Conduct in-person interviews with the family and conduct home visits Keep family information file and database up to date/current Document all interactions with the family in Salesforce database Facilitate family grants
  • collect bills/invoices and complete disbursement expense forms.
Invoices will be processed by another individual. Provide ongoing follow-up and check-ins with families to support their well-being and ensure continued care and connection Assess overall well-being of families and create goals Coordinate and provide referrals to other local community resources for families Act as a liaison between families and available resources (e.g., healthcare providers, other local community services, etc.) Work with families to coordinate transportation through other local agencies Participate in fundraising activities Attend events to promote community awareness Promote Hearts Aligned as appropriate Qualifications Bachelor's Degree preferred Bilingual in Spanish required Candidate should have excellent written and verbal communication skills in English and Spanish Case management experience of 3-5 years preferred Computer skills are required, including the use of Google Drive and Salesforce databases The candidate should have excellent organizational and time management skills. Experience working with families in need is required Must have reliable transportation Employment will be out of the organization's Santa Barbara office. Compensation $25
