In this vital role, you will provide personalized, respectful support to patients nearing the end of life, ensuring comfort, dignity, and quality of life. Your dedication will help families navigate challenging times with empathy and professionalism, making a meaningful difference every day. This paid position offers an opportunity to work in a dynamic environment where patient-centered care is at the forefront of everything we do. We are hiring in all Counties and are are holding Interviews week. Must pass Background check. Responsibilities Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility to promote comfort and independence. Monitor and record vital signs, including blood pressure, temperature, pulse, and respiration rates, accurately documenting all patient information in electronic health records (EHR systems). Support medication administration under supervision, ensuring proper techniques and adherence to safety protocols. Provide emotional support and companionship to patients and their families, respecting cultural and individual preferences. Maintain a safe environment by utilizing equipment like Hoyer lifts and adhering to aseptic techniques during patient care procedures. Collaborate with interdisciplinary teams including nurses, physicians, social workers, and therapists to coordinate comprehensive hospice services. Follow all HIPAA regulations and medical documentation standards to protect patient confidentiality and ensure compliance with healthcare laws. Skills Proven experience in hospice care or related fields such as long-term care, assisted living, or nursing homes. Familiarity with EMR (Electronic Medical Records) systems like Epic or Cerner for accurate documentation. Knowledge of medical terminology, anatomy, physiology, and vital signs monitoring. Skilled in infection control practices including aseptic technique and infectious disease care protocols. Experience working with individuals with developmental disabilities or dementia care needs such as Alzheimer's or memory care. Ability to perform complex patient care tasks including catheter care, phlebotomy support, and use of medical equipment like Hoyer lifts. Strong communication skills for effective patient service delivery and team collaboration in diverse healthcare settings including inpatient, outpatient, or home care environments.
