Position Requirements:
For CNA's, must be listed on the state registry and be approved with skills competency signed off by registered nurse. For PCA's, must have completed state approved PCA course with a certificate showing hours of instruction completed and must have skills competency evaluation signed off by a registered nurse. If not PCA certified upon hire, then rate of pay is $13.00 and hour. Must be at least 18 years of age.
Position Description:
Duties are outlined as, but not limited to, the following: A. Personal Care
  • Provide assistance to residents with activities of daily living (ADLs) according to the care plan (toileting, ambulation, eating, transferring, dressing, and personal hygiene)
  • Provide and ensure bath lists are followed assisting with bathing documenting on the bath log
  • Ensure restraints and assisted devices are properly utilized and documented
  • Ensure 15/30 minutes checks are complete and documented.
  • Ensure nursing notes and logs are accurate and documented in a timely manner
  • Ensure residents' hands and feet are clean, infection free
  • Ensure nails are appropriately managed
  • Ensure skin is moisturized
  • Ensure residents are dressed properly according to season and weather
  • Provide assistance to residents with skills in "self-help" activities and ADLSs to maximize their independence/freedom of movement (assistance with brushing teeth, buttoning buttons, etc)
  • Provide assistance with all personal care tasks as needed, requested or required
  • Respond to hand bell, call bells or other signaling for assistance within 2 minutes (response may vary depending on situation and you may call for assistance if necessary)
  • Assist residents to room if observed sleeping in the wheelchair, on couch or in hallway chairs
  • Assist resident with keeping clothing, closets and rooms neat and orderly (ask housekeeping for assistance if necessary)
  • Provide residents with clean towels and wash clothes (do not allow them to get WB linen alone)
  • Assist with bed making
  • Assist with laundry when necessary (do not utilize laundry room until proper training)
  • Assist with using, emptying and cleaning resident equipment (chairs, beds, urinals, bedside commodes, bed pans, wash basins, walkers, wheelchair, etc) B. Meals
  • Assure residents receive snacks
  • Assist with mealtimes and ensure someone is present while residents are in dining area
  • Ensure residents on front porch during spring/summer days are not overheated and push fluids C. General Care
  • Orient and assist new residents in the community
  • Assist with transport when indicated
  • Assist with activities and ensure all residents are aware of activity times throughout the day
  • Maintain accurate and detailed documentation as required
