  • Part-time, Full-time, Per diem Job Overview We are seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to join our team as a W-2 employee.
In this vital role, you will provide personalized, high-quality care to individuals in their homes or assisted living environments. Your expertise will help promote independence, comfort, and safety for our clients, ensuring they receive the attentive care they deserve. This position offers an engaging opportunity to make a meaningful difference in the lives of those with diverse health needs, including seniors, individuals with disabilities, and those requiring memory or hospice care. Duties Perform patient observation to monitor health status and report changes promptly Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Administer medications and manage medication records in compliance with HIPAA regulations Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for accurate documentation of patient care Conduct vital signs assessments including blood pressure, pulse, temperature, and respiration rates Provide specialized care for patients with Alzheimer's disease, dementia, or memory impairments Assist with catheter care, wound care, hospice support, and other medical procedures as trained Operate equipment such as Hoyer lifts safely to transfer clients with mobility challenges Support patients with meal preparation and nutritional needs while maintaining cleanliness and hygiene standards Perform light housekeeping tasks including laundry and cleaning to ensure a safe living environment Work closely with healthcare teams to coordinate patient care plans and ensure compliance with medical terminology standards Qualifications Valid Certified Home Health Aide (CHHA) certification required Prior experience in nursing homes, assisted living facilities, or home health settings preferred Strong knowledge of patient care techniques including assisting with ADLs and dementia care Experience working with individuals with developmental disabilities or disabilities in general is highly valued Familiarity with EMR/EHR systems for medical documentation and record keeping CPR certification required; additional certifications such as hospice care or catheter care are a plus Ability to communicate effectively and demonstrate empathy in patient interactions Knowledge of HIPAA regulations to ensure patient privacy and confidentiality Long-term care experience is advantageous; experience in home & community care settings is preferred Join us in delivering compassionate caregiving that transforms lives!
