Job Description

Overview The Case Manager provides high-quality psychotherapy and case management services to support patients and families throughout the continuum of care. Responsibilities include conducting assessments, diagnoses, and treatment planning; facilitating individual, group, and family therapy; and assisting patients and families in understanding and adjusting to treatment. This role also leads discharge planning by identifying needs at admission, coordinating with the care team, arranging aftercare services, and maintaining follow-up to ensure safe transitions. As an integral member of the interdisciplinary team, the Case Manager collaborates with physicians, staff, and community resources, maintains partnerships with service agencies and residential facilities, and ensures completion of required documentation. The role may provide clinical supervision to staff and, per DPU requirements, may serve as Director of Social Services for the program.

Position Type/Expected Hours of Work:
This is a full-time position, and hours of work and days are subject to the needs of the Program which operates on a 24 hour; seven days a week basis. Please refer to Program or Hospital attendance policy for additional direction on shifts, holidays and attendance.
Essential Duties:
Conducts assessments and reassessments of patient needs using appropriate clinical tools (e.g., Geriatric Depression Scale, Zung, SLUMS, alcohol screening tools), accurately documents findings, and develops individualized treatment and discharge plans based on symptom presentation, referral input, and all available data Actively participates as a member of the interdisciplinary and treatment teams, contributing to the assessment of patient progress and communicating results and trends to the supervisor and team. Coordinates and completes timely discharge planning in collaboration with the treatment team, physician, patient, and family. Ensures all discharge forms are accurately completed by the date of discharge. Documents challenges in implementing the discharge plan, including actions taken and the effectiveness of those actions. Ensures patients are discharged to a safe environment and coordinates necessary referrals, resources, and follow-up careincluding medical and psychiatric appointmentsto support continuity of care Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, SLUMS, alcohol screening tool(s) etc. Provides guidance, education, and support to patients and families, addressing financial or other barriers that may impact discharge Works with the treatment team to resolve any issues that may delay or interfere with safe discharge Reviews and discusses the discharge plan with patients and families, obtaining input, explaining the plan, and securing signatures for involvement in treatment planning. Maintains accurate, timely, and comprehensive documentation in patient charts, including services provided, contacts with patients, families, physicians, and agencies. Ensures complete recording of diagnoses, comorbidities, and complications, Completes progress notes and assessments per program procedures Submits required information to the incident reporting system Utilizes data to generate key statistical and program reports. Provide ongoing education and guidance to patients and families as needed. Offer appropriate resources and support before, during, and after treatment Assist patients in obtaining necessary equipment through agency contacts while maintaining cooperative relationships. Communicates with referral sources including, family members, legal system, psychiatrists, primary care physicians, and other treatment facilities Promotes and develops positive, professional interaction with patients and their families, peers, leadership, clients, etc. Conducts oneself ethically and professionally at all times, maintaining appropriate clinical boundaries and demonstrating respect for patient rights Adheres to all relevant Mental Health policies, procedures, and standards, including state and federal confidentiality laws as well as Horizons confidentiality policies and Code of Conduct Conducts group therapy sessions as directed, including weekends demonstrating strong theoretical knowledge of group therapy principles, psychopathology, and effective therapeutic techniques Utilizes performance feedback to improve effectiveness, identifies areas for improvement, participates in performance improvement processes, and addresses or reports issues and errors to the Program Director Works sufficient hours to ensure effective performance. Coordinates work hours with supervisor Addresses issues in a timely manner including crisis and higher level of care referrals Attends Unit and Hospital meetings as directed Assists in submission of reports and requests for documents from various regulatory agencies Completes and maintains all required state/regulatory, hospital and company competencies and training expectations.

Responsibilities Required Education/Licensure:
Masters Degree from an accredited college in Social Work Licensed eligible Social Worker or Therapist in a field which includes a practicum in a health or human service agency
Preferred Education/Licensure:
LSW, LPC or NCC within 3 years of hire
Preferred Experience:
Minimum three years of experience in a similar position and/or industry
Required Experience:
Proficient in software applications Excellent written and verbal communication and presentation skills Ability to read and comprehend moderate to complex instructions and correspondence Ability to write detailed correspondence Ability to effectively present information in one-on-one and small group situations to patients and their families, Program leadership, employees, clients, providers, vendors, etc.
Work Environment:
This job operates in a professional office environment and/or clinical environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines. This position may have contact with patients who have moderate to severe psychiatric concerns and/or behavioral issues. There is the possibility this position could be exposed to violent behavior from patients and/or behavioral issues as part of their daily duties.
