Full Time, M-F 8a-5p Req # 47587 The Clinical Coordinator is a Registered Nurse, a member of the patient care team within the Department of Home Care, and reports to the Clinical Director of Home Care. This position is responsible for coordinating IV referrals, ordering medications and supplies through IV companies or FGH Pharmacy. Responsible for gathering IV-related QAPI data and submitting timely. Participates in Infection Control activities and reporting. Supports the agency by facilitating accurate transfer and discharge information. This role is pivotal in ensuring valid, timely, accurate transfer and discharge OASIS responses as well as Transfer and Discharge summaries in compliance with state, federal, and accrediting body regulations. Demonstrates ability to effectively communicate with physicians and other providers. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patient's served; demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. Must possess sound decision-making abilities.
Performance Expectations:
Demonstrate ability to oversee medication administration in the home setting by peripheral and IV administration. Demonstrate ability to enter all patients charting in EMR according to state and federal regulations. Coordinate and execute transfer and discharge regulatory requirements including OASIS validation and documentation in EPIC. Use SHP (Strategic Healthcare Programs) analysis of OASIS response to address all informational and accuracy alerts. Collaborate with the interdisciplinary team to identify and eliminate transfer and discharge OASIS discrepancies. Offer education to clinicians related to any noted negative trends or individual issues with accurate OASIS responses. Escalate concerns related to delays in discharge or inaccurate OASIS responses to the Patient Clinical Manager and/or Clinical Director. Maintain documentation compliance with hospital policy. Trend data related to discharge OASIS errors/corrections. Follow-up with requested OASIS changes. Communicates effectively with others in an appropriate and timely manner. Adheres to and promotes the established values of the organization, i.e., customer service, safety, and compliance standards. Demonstrates the ability to promote a culture of caring through compassionate care, teamwork, and courtesy in all interactions with customers, both internal and external. Demonstrates financial responsibility by demonstrating good decision making regarding use of FGH pharmacy for patients, using supplies in a cost-efficient manner, and ensuring that patients are discharged in a timely manner. Qualifications:
Education/Skills Graduate of a recognized accredited non-online RN program required. RN with BSN preferred
Work Experience:
Minimum one year medical/surgical nursing experience required, and at least one year of experience in home health nursing preferred. Certification/Licensure-DUE
UPON HIRE
Licensed RN able to practice within the State of MS as an advanced practice Nurse. Valid Drivers License Proof of automobile insurance Additional Certification/Licensure - Obtained based on required timeframe below Basic Life Support Within 30
