Responsibilities Assist patients with activities of daily living, including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts and other assistive devices Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration rates, ensuring accurate medical documentation Support medication administration under supervision, following strict aseptic techniques and HIPAA guidelines for patient privacy Provide specialized care for individuals with dementia, Alzheimer's disease, memory impairments, or behavioral health needs Collaborate with interdisciplinary teams in settings such as long-term care facilities, hospitals—including ICU and trauma centers—hospice, outpatient clinics, and home care environments Maintain a safe environment by adhering to infection control protocols and practicing proper aseptic techniques during procedures like catheter care and wound management Document patient information accurately within Electronic Health Record (EHR) systems such as Epic or Cerner to ensure seamless communication across care teams Skills Proven experience working with diverse populations including seniors, pediatrics, individuals with disabilities, or those requiring behavioral health support Familiarity with medical documentation and patient records management Knowledge of physiology, anatomy, medical terminology, and infection control practices necessary for high-quality patient care Ability to perform phlebotomy procedures when required and manage complex medical tasks in acute or sub-acute care settings Experience in specialized areas such as post-acute care environments is highly desirable Strong caregiving skills with a focus on geriatrics, hospice care, pediatric care, or home & community-based services Certification in Basic Life Support (BLS) and understanding of HIPAA compliance to ensure patient safety and confidentiality Join our team to provide compassionate care that transforms lives!
