JOB DESCRIPTION I. POSITION TITLE
CERTIFIEDNURSING ASSISTANT II. PURPOSE OF YOUR JOB POSITION
The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by your supervisors.III. DELEGATION OF AUTHORITY

Administrative Functions a. Use the wristband to identify residents before administering treatments, serving meals, etc., as necessary. b. Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner. c. Use only authorized abbreviations established by this facility when recording information. d. Report all changes in the resident's conditions to the LPN/Nurse Manager/Supervisor as soon as practical. e. Report all accidents and incidents you observe on the shift that they occur. Admission, Transfer, and Discharge Functions a. Ensure that the resident's room is ready for receiving the resident (i.e., bed name, name tags up, admission kit available, etc.). b. Greet residents and escort them to their room. c. Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate. d. Make resident comfortable (i.e., put to bed, get water, etc.). e. Store resident's clothing. f. Assist residents with packing their personal possessions when they are being transferred to a new room, or when being discharged. g. Transport residents to new rooms or to the receiving area. h. Assist with loading/unloading residents from vehicles as necessary. Personnel Functions a. Perform all assigned tasks in accordance with our established policies and procedures, and as instructed by your supervisors. b. Follow work assignments, and/or work schedules in completing and performing your assigned tasks. c. Cooperate with inter-departmental personnel, as well as other facility personnel to ensure that nursing services can be adequately maintained to meet the needs of the residents. d. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift. e. Meet with your shift's nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or the improvement of services. f. Report all complaints and grievances made by the resident. Personnel Functions (continued..) g. Notify the facility when you will be late or absent from work. h. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents. Personal Nursing Care Functions a. Participate in and receive the nursing report upon reporting for duty. b. Follow established policies concerning exposure to blood/body fluids. c. Provide nursing services to resident as indicated on resident care profile. d. Assist residents with daily dental and mouth care (i.e., brushing teeth/dentures, oral hygiene, special mouth care, etc.). e. Assist residents with bath functions (i.e., bedbath, tub or shower bath, etc.) As directed. f. Assist residents with dressing/undressing as necessary. g. Assist residents with hair care functions (i.e., combing, brushing, shampooing, etc.). h. Assist residents with nail care (i.e., clipping, trimming, and cleaning the fingernails). (Note:
does not include diabetic residents.) I. Shave residents (male or female), as needed. j. Keep residents dry (i.e., change gown, clothing, linen, etc., when it becomes wet or soiled). k. Change bed linens. Keep linens tight to avoid wrinkles from forming under the resident. l. Make beds (occupied and unoccupied). m Put extra covers on beds as requested. n. Position resident in correct and comfortable position. o. Turn and position residents utilizing the universal clock or a resident specific turning schedule. p. Assist resident with bowel and bladder functions (i.e., take to bathroom, offer bedpan/urinal, portable commode, etc.). Maintain toileting schedule, as written on CNA accountability sheet. q. Maintain intake and output records as instructed. r .Keep incontinent residents clean and dry. s .Check and report bowel movements and character of stools as instructed. t. Prepare and give enemas. Report results as instructed. u. Collect specimens as instructed (i.e., urine, sputum, stools, etc.). v. Assist residents in preparing for medical tests (i.e., lab work, e-ray, therapy, dental, etc.). w. Assist residents in preparing for activity and social programs (i.e., church services, parties, visitors, etc.). x. Assist in transporting residents to/from appointments, activity and social programs, etc., as necessary. y. Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. z. Assist residents to walk with or without self-help devices as instructed. aa Perform restorative and rehabilitative procedures as instructed. bb Change colostomy bags, binders, etc., as instructed. cc. Provide eye and ear care (i.e., warm compresses, care of eyeglasses and hearing aides, etc.) as instructed. dd. Assist with the application of non-sterile (moist and dry) warm/cold compresses. ee. Assist in preparing the resident for a physical examination. ff. Weigh and measure residents as instructed. gg. Measure and record temperatures, pulse, and respirations (TPRs), as instructed. hh. Answer resident calls promptly. ii. Ensure that residents who are unable to call for help are checked frequently. jj. Check each resident routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes. kk. Assist with the care of the dying resident. ll. Provide post-mortem care as instructed. Special Nursing Care Functions a. Observe and report the condition of pressure areas and any changes in skin conditions to prevent pressure ulcers (bedsores). b. Report injuries of an unknown source, including skin tears. c. Provide daily indwelling catheter care. d. Provide perineal care daily and as needed. e. Perform non-sterile (clean) vaginal irrigation/douches. f. Assist with the application of slings, binders, etc. g. Give tepid sponge baths. h. Provide daily Range of Motion Exercises, as ordered. Record data as instructed. I. Turn bedfast residents as least every two (2) hours. Special Nursing Care Functions (continued..) j. Perform special treatments as instructed. k. Report new or unusual resident behaviors. l. Turn all medications found in the resident's room/possession over to the LPN/Nurse Manager/Supervisor. m. Provide residents with Reality Orientation as instructed. n. Watch for and report any change in room temperature, ventilation, lighting, etc. o. Document resident care provided on the CNA Accountability Record, as instructed. Food Service Functions a. Prepare residents for meals (i.e., take to bathroom, wash hands, comb hair, raise bed, position tables, place bibs, take to/from dining room, etc.). b. Serve food trays. Assist with feeding as indicated (i.e., cutting foods, feeding, assist in dining room supervision, etc.). c. Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.). d. Record the resident's food/fluid intake. Report changes in the resident's eating habits. e. Keep residents' water pitchers clean and filled with fresh water (on each shift), and within easy reach of the resident. f. Serve between meal and bedtime snacks and/or nourishments. g. Perform after meal care (i.e., remove trays, clean resident's hands, face, clothing, take to bathroom, brush teeth, clean dentures, etc.). h. Check rooms for food articles (i.e., food in proper container, unauthorized food items, etc.). Staff Development a. Attend and participate in scheduled training and educational classes to maintain current certification as a Nursing Assistant. b. Attend and participate in scheduled orientation programs and activities. c. Attend and participate in annual OSHA and CDC in-service training programs for hazard communication, TB management, and bloodborne pathogens standard. d. Attend and participate in advance directive in-service training programs as scheduled. Safety and Sanitation a. Restrain residents in chair/bed as instructed. b. Check restrained residents to ensure safety. c. Release restraints at least every two (2) hours for range of motion exercises, taking to bathroom, etc. d. Document restraint release and release-period activity on CNA accountability record. e. Participate in appropriate in-service training programs prior to performing tasks that involve potential exposure to blood/body fluids. f. Wash hands before and after performing any service for the resident. g. Keep the nurses' call system within easy reach of the resident. h. Immediately notify the LPN/Nurse Manager/Supervisor of any resident leaving/missing from the facility. I. Follow established safety precautions in the performance of all duties. j. Keep the resident's personal possessions off the floor and properly stored. k. Keep floors dry. Report spills immediately. l. Keep excess supplies and equipment off the floor. Store in designated areas. m. Clean spills from wheelchairs, walkers, etc., as needed. n. Clean, disinfect, and return all resident care equipment to its designated storage area after each use. o. Perform routine housekeeping duties (i.e., clean bed rails, overbed table, night stand, etc., that relate to nursing care procedures.). p. Before leaving work area for breaks, or at the end of the work day, store all tools, equipment, and supplies. q. Report all hazardous conditions and equipment to the LPN/Nurse Manager//Supervisor immediately. r. Report all safety violations. s. Follow established smoking regulations. Report all violations. t. Report any communicable or infectious disease to the Director of Nursing Services and/or to the Infection Control Coordinator. u. Follow established disease specific precautions and procedures. v. Wash hands before entering and after leaving an isolation room/area. w. Follow established procedures in the use and disposal of personal protective equipment. x. Store and transport resident personal care items in the receptacle provided by the facility. y. Report missing/illegible labels and MSDSs to your supervisor. Equipment and Supplies a. Use only the equipment you have been trained to use. b. Operate all equipment in a safe manner. c. Use only the equipment and supplies necessary to do the job. Do not be wasteful. d. Report defective equipment to the LPN/Nurse Manager/Supervisor. e. Inform the LPN/Nurse Manager/Supervisor of your equipment and supply needs. Care Plan Functions a. Review the Resident's Care Profile daily to determine if changes in the resident's daily care routine have been made on the care plan. b. Inform the LPN/Nurse Manager/Supervisor, of any changes in the resident's condition so that appropriate information can be entered on the resident's care plan. c. Ensure residents' CNA accountability record accurately reflects the care outlined on the resident care profile is being delivered. Resident Rights a. Maintain the confidentiality of all resident care information. b. Ensure that you treat all residents fairly, and with kindness, dignity and respect. c. Ensure that all nursing care is provided in privacy. d. Knock before entering the resident's room. e. Report all complaints and grievances made by the resident to the LPN/Nurse Manager/Supervisor. f. Report allegations of resident abuse and/or misappropriation of resident property. g. Honor the resident's refusal of treatment request. Report such requests to your supervisor.VI. WORKING CONDITIONS
a. Works throughout the nursing service area (i.e., nurses' stations, resident rooms, etc.). b. Moves intermittently during working hours. c. Is subject to frequent interruptions. d. Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances. e. Is subject to hostile and emotionally upset residents, family members, personnel, and visitors. f. Communicates with nursing personnel, and other department personnel. g. Works beyond normal working h ours and on weekends and holidays when necessary. h. Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.) i. Attends and participates in continuing educations programs. j. Is subject to injury from falls, burns from equipment, odors, etc., throughout the work day, as well as to reactions from dust, disinfectants, and other air contaminants. k. May be subject to the handling of an exposure to hazardous chemicals. l. May be subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses.VII. EDUCATION
Must possess as a minimum a twelfth (12) grade education or equivalent.VIII. EXPERIENCE
Must be a licensed Certified Nursing Assistant in accordance with laws of the State of New York.IX. SPECIFIC REQUIREMENTS
a. Must be able to read, write, speak and understand the English language. b. Must possess the ability to make independent decisions when circumstances warrant such action. c. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. d. Must possess the ability and willingness to work harmoniously with other personnel. e. Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents. f. Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices. g. Must be able to relate information concerning a resident's condition. h. Must not pose a direct threat to the health or safety of other individuals in the workplace. i. Must maintain full vaccination status againstCOVID-19
virus, unless given medical exemption with accommodation .X. PHYSICAL AND SENSORY REQUIREMENTS
(With or Without the Aid of Mechanical Devices) a. Must be able to move intermittently throughout the workday. b. Must be able to speak and write the English language in an understandable manner. c. Must be able to cope with the mental and emotional stress of the position. d. Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met. e. Must function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies. f. Must meet the general health requirements set forth by the policies of this facility which include a medical and physical examination. g. Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the facility. h. Must be able to push, pull, move and/or lift a minimum of 75 pounds to a minimum height of 3 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet. i.
