Community Health Worker

Responsibilities:

• Conduct outreach and engage with community members to provide health education and resources
• Assist individuals in navigating the healthcare system, including Medicare enrollment and understanding medical terminology
• Provide support and counseling for individuals struggling with addiction or other health-related issues
• Collect data and maintain accurate records of client interactions and outcomes
• Collaborate with healthcare professionals to develop and implement health promotion programs
• Conduct public speaking engagements to raise awareness about health issues and promote community wellness
• Utilize motivational interviewing techniques to empower individuals to make positive health choices

Requirements:

• Strong knowledge of Medicare and experience working with diverse populations
• Excellent communication skills, both written and verbal, with the ability to effectively engage with individuals from various backgrounds
• Familiarity with medical terminology and the ability to explain complex medical concepts in a clear and understandable manner
• Experience in addiction counseling or related fields is highly desirable
• Ability to work independently, prioritize tasks, and manage time effectively
• Strong organizational skills with attention to detail for accurate data collection and record keeping
• Compassionate, empathetic, and non-judgmental approach to working with individuals facing health challenges
