  • CStar provides person-centered, strength-based support to adults navigating behavioral health and substance use challenges.
In this role, you'll partner with individuals in residential and community settings to build skills, foster independence, and support progress toward personally defined wellness and recovery goals. Key Responsibilities Deliver individualized, strengths-based services using a motivational, stage-matched approach Partner with individuals to identify goals and develop and update Wellness Plans Provide holistic support across the Eight Dimensions of Wellness, including physical and mental health Coach and build skills in daily living, communication, employment/education, and independent living Support individuals in accessing community resources such as housing, financial assistance, and healthcare Collaborate with integrated care teams and natural supports to ensure coordinated, whole-person care Assist with care transitions, including hospital discharge planning and follow-up support Accompany individuals to appointments and provide ongoing engagement and advocacy Maintain timely, accurate documentation and meet service expectations Provide transportation as clinically indicated Education, Skills, Knowledge and Expertise High School/GED required Associate's/Bachelor's degree in a human service-related field preferred
WORK EXPERIENCE/TRAINING/ADDITIONAL REQUIREMENTS
Must qualify for position based on overall CSS requirements in at least one of the options listed: Qualified addiction professional; Bachelor's degree in a human services field from a college or university included in the U.S. Department of Education's database of accredited schools at http:/ope.ed.gov/accreditation ; Any four-year degree or combination of higher education and qualifying experience; Four years of qualifying experience; or Associate of Applied Science in Behavioral Health Support degree as designated by the department. Qualifying experience must include delivery of service to individuals with mental illness, substance use disorders, or developmental disabilities. A valid, unrestricted driver's license and agency established minimum automobile coverage required Lead with empathy and believe in a recovery-oriented, person-centered approach Naturally focus on strengths and possibilities rather than limitations Enjoy working both independently and as part of a collaborative care team Are adaptable and comfortable working in residential and community-based settings Communicate effectively with diverse individuals, families, and providers Are organized and able to balance documentation with meaningful client engagement About Compass Health Network About Compass Health Network Compass Health Network is a nonprofit organization dedicated to delivering accessible, high-quality care throughout Missouri.
