Overview The Field Supervisor in a home service agency oversees field operations, ensuring high-quality service delivery to clients and effective management of caregivers in the field. This role includes conducting site visits, monitoring caregiver performance, ensuring compliance with agency policies and state regulations, and addressing client and caregiver concerns. The Field Supervisor acts as a liaison between clients, caregivers, and the administrative team, ensuring continuity of care and operational excellence. Due to diversity in our clients, we are looking for a fluent spanish speaker.
Essential Responsibilities:
Caregiver supervision: Monitor and evaluate caregiver performance through regular field visits and direct observation of service delivery. Provide training, coaching, and support to caregivers to ensure adherence to agency standards and client-specific care plans. Address caregiver concerns and resolve performance issues in collaboration with the HR department. Assist with onboarding and orientation of new caregivers.
Client Care Monitoring:
Conduct periodic client visits to assess service quality, client satisfaction, and adherence to care plans. Address client concerns promptly, ensuring effective resolution and high client satisfaction. Communicate any changes in client needs or care plans to the appropriate staff and caregivers.
Compliance and Quality Assurance:
Ensure that field operations comply with all agency policies, state regulations, and industry standards. Complete and document site visit reports, incident reports, and quality assurance reviews. Monitor caregiver compliance with required certifications, training, and documentation.
Scheduling and Coordination:
Assist with caregiver scheduling to ensure adequate coverage for client needs, especially during absences or emergencies. Coordinate with the administrative team to address last-minute scheduling changes or conflicts.
Emergency Response:
Respond to emergencies in the field, providing immediate support to clients and caregivers as needed. Act as the primary point of contact for urgent issues outside regular office hours, as required.
Documentation and Reporting:
Maintain accurate and up-to-date records of site visits, caregiver evaluations, and client feedback. Provide regular reports to management on field operations, quality metrics, and areas for improvement. Participate in regular staff meetings and contribute to operational planning and process improvement initiatives.
Coordinate and Schedule Start of Care Visits:
Work with new clients and their families to arrange initial home visits to begin services. This involves gathering all necessary information beforehand and conducting a thorough assessment of the client's needs to ensure an effective start to care.
Prepare Client Start-of-Care Documentation:
Assemble all required documents into an organized binder for each client. This includes care plans, assessment forms, consent forms, emergency contact details, and other relevant records. These binders serve as essential tools for caregivers, providing quick and comprehensive access to client information.
Maintain Hard Copies of Client Documents:
Securely file physical copies of all client-related documentation in accordance with company policies and regulatory requirements. Properly organized files ensure compliance and ease of access during audits, quality checks, or follow-up visits. Update Client Profiles in
Digital Systems:
Accurately input and maintain client data within the company's electronic system. This includes personal details, medical information, care plans, and progress notes to provide a centralized and accessible resource for all team members involved in the client's care.
Skills and Competencies:
Excellent communication and interpersonal skills to work effectively with clients, caregivers, and internal staff. Strong organizational and time management skills to handle multiple responsibilities and priorities. Ability to make sound decisions under pressure and in emergency situations. Proficiency in using scheduling software, client management systems, and basic office applications. Empathy and a customer-focused approach to problem-solving.
Working Conditions:
Frequent travel to client homes, caregiver locations, and community sites. May involve exposure to diverse home environments and varying client needs.
Qualifications:
