Are you energized by creating extraordinary customer experiences, bringing value, building relationships and educating potential clients on the benefits of Home Care services through Right at Home? Do you have oversight to manage client accounts and ensure they are getting quality care and have an extraordinary experience? If so, Right at Home has the opportunity for you! The Client Care Manager is responsible for educating potential clients on Right at Home's services to bring value to families inquiring for their loved one. The CCM is also responsible for ensuring the quality of care that is being provided in the home.
Responsibilities:
Client Care Manager's play an integral role in helping Right at Home improve the lives of as many people as possible in our community. Immediately answers the phone with the express intent of improving the quality of life for the caller. Relies on empathy and asks open ended questions to learn and understand a family's care needs. Shares helpful community resources when needed. Schedules in-home assessments and arranges care on an agreed upon start date. Conducts Home Assessments and assists in developing, modifying, and maintaining the client care plan, creating extraordinary client experiences, and communicating with clients, families, care partners and referral sources. Plays an integral role in interfacing with clients and families to ensure safe, appropriate, and meaningful care is being provided by conducting regular Supervisory Visits Announced and Unannounced. Oversees the Quality of Care and addresses any client concerns and passes any feedback, positive or negative regarding the Care Pro on to the Recruiter. Conducts Care Pro/Client Introductions and training when requested. Provides direct care with clients as needed. Always ask for client feedback and solicit google reviews to share experiences with others in the community. Builds and maintains strong relationships with community partners including hospitals, clinics, assisted living communities, skilled nursing facilities, social workers, case managers, and other healthcare professionals. Regularly visits referral partners and community organizations to strengthen existing relationships and develop new connections that support client care and community awareness of Right at Home services. Represents Right at Home at community events, networking opportunities, health fairs, and partner meetings as assigned. Conduct Random Drug Testing periodically during supervisory visits. Will assist with answering the phones and scheduling shifts as needed.
EDUCATION / SKILLS / ABILITIES / AVAILABILITY
High school graduate Associate degree in social work or public service preferred Active CNA license in Nebraska Basic office and computer skills and organizational abilities. Excellent interpersonal relations abilities. Excellent telephone skills. Excellent interpersonal relations abilities. Excellent telephone skills. Knowledge of common medical terminology. Able to work independently, demonstrating sound judgment. Read, write, speak, and understand English as needed for the job. Have a valid driver's license and insurance
