Job Description

Job Overview We are seeking a proactive and compassionate Care-Coordinator to join our dynamic homecare agency team. In this vital role, you will serve as the primary point of contact for clients, caregivers, healthcare providers, and insurance companies. Your energetic approach will ensure seamless coordination of homecare services, optimize client satisfaction, and uphold the highest standards of quality care. You are calm under pressure and can quickly pivot to handle caregiver call-offs or client concerns. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring personalized support while fostering a collaborative and efficient work environment. Duties Ensures all shifts are covered by coordinating with our overseas scheduler and recruiter, together wiht the support of our local team members Supports morning office on-call hours from home 6am-9am Must be willing and able to take call-off shifts with clients and work as an experienced caregiver. Capped at 15 hours per week. Priority is to confirm care from our caregiver roster. Manage and coordinate client care plans, ensuring timely scheduling of home visits and services Communicate effectively with clients, families, healthcare providers, and caregivers via phone, email, and internal messaging systems Maintain accurate and detailed electronic health records (EHR) to document care activities, assessments, and updates Oversee insurance processes including third-party billing, health insurance verification, and revenue cycle management to ensure smooth reimbursement workflows Handle data entry tasks with precision to support client records, scheduling logs, and billing information Provide exceptional customer service by addressing client inquiries promptly and professionally Collaborate with internal teams to streamline schedule management and optimize resource allocation for maximum efficiency Support Marketing efforts by visiting local facilities. Will visit 5-8 local facilities in the territory to secure client leads. Requirements CNA and experienced caregiver with at least two years supervisory background Proven administrative experience within healthcare or related fields with a focus on customer service and data management Proficiency in Google applications such for documentation and communication purposes Knowledge of managed care systems, health insurance procedures, and third-party billing processes. AxisCare knowledge is a plus. Familiarity with electronic health records (EHR) management systems and medical terminology to accurately document patient information Strong phone communication skills coupled with excellent writing abilities for clear documentation and client correspondence Ability to manage schedules efficiently while adapting to changing priorities in a fast-paced environment Understanding of revenue cycle management processes including billing cycles, claims submission, and payment follow-up Attention to detail in data entry tasks such as typing accuracy and record keeping Join us in delivering compassionate homecare services that empower clients to live independently!
