Coverage Area:
Colorado Springs Schedule:
Hybrid (Tuesdays & Thursdays—Lakewood office, Mon/Wed/Fri—field visits)

  • Collaborative culture where everyone's voice matters What You'll Do As an Unskilled Services Coordinator , you will:
Advocate & Coordinate:
Work with case management entities to develop personalized care plans for PASCO clients, ensuring all needs are met.
Ensure Quality Care:
Conduct on-site supervisory visits and provide feedback on Personal Care Workers (PCWs) and Homemakers.
Support Clients & Caregivers:
Act as a trusted resource to help families navigate Medicaid and waiver services .
Collaborate Across Teams:
Work with PASCO departments, including Medical Records, Prior Authorization, HR, and Intake to streamline processes.
Maintain Compliance & Documentation:
Keep records updated in Electronic Health Record (EHR) systems and ensure compliance with PASCO and state policies.
Build Relationships:
Establish strong community partnerships and act as a go-to support for clients, families, and caregivers . What You Bring to the Team Experience in Case Management or Client Care Coordination
  • Familiarity with Medicaid, Home Healthcare, or IHSS is a big plus! Customer Service & Relationship Management Skills
  • At least 2 years of experience in client-facing roles, ensuring high levels of satisfaction. Tech-Savviness
  • Proficiency in Microsoft Office Suite, Outlook, and Electronic Health Record (EHR) systems. Self-Motivation & Adaptability
  • Ability to work independently and in a fast-paced, evolving healthcare environment. Willingness to Travel
  • Conduct home visits in the Lakewood area to ensure high-quality care.
