Full Job Description Primary Duties:

Primary Home Care (PHC) Program assessments, reassessments, and scheduled PHC Program supervisory home visits. In-home attendant orientations and competency training for Primary Home Care (PHC) attendants.

Minimum requirements:

• Minimum two years of full-time study at an accredited college or university.

OR:

an equivalent amount of demonstrated supervisory work experience in a health care facility, agency, or community-based agency for each required year of college

AND:

• Primary Home Health Care agency experience (preferred)
• Bilingual (English/Spanish)

Computer Skills:

Microsoft Office Suite

• Excellent interpersonal, communication problem solving, and customer service skills;

Valid Drivers License/Good Driving Record Job Type:
