JOB SUMMARY
The Client Navigation Specialist will be the central point of contact for clients seeking permanent housing. This involves screening clients and developing case plans to determine their needs, to ensure they are fully prepared for the responsibilities of being a successful tenant. The Specialist will be a role model and mentor, encouraging clients to make weekly progress toward their case plan goals, while demonstrating positive ethics and boundaries that ensure respect for the rights, dignity and safety of clients while maintaining objectivity and professionalism.
JOB RESPONSIBILITIES
Conduct intake and enrollment activities with eligible clients, assist clients with other program eligibility documentation, complete project intake forms, authorization and enroll into Case Management System. Conduct CES assessment to determine appropriate permanent housing intervention and service needs Assist clients with obtaining income and/or establishing benefits, with locating and securing employment and volunteer and/or educational activities, assist with basic needs, life skills, community participation, improving and/or maintaining daily independent living skills, social/leisure, personal hygiene skills, budgeting and money management, legal issues and making appropriate referrals to overcome barriers to accessing and obtaining permanent housing, and provide transportation as needed. Assist with temporary housing referrals until permanent housing can be secured. Develop professional relationships with clients and implement interventions with an approach emphasizing Client choice, harm-reduction, and psychosocial rehabilitation within a setting guided by trauma-informed care, housing-first principles, and professional boundaries. Assist clients to establish eligibility for subsidized housing including the collection of all required documents and completion of all applications. Maintain a caseload of project-based and/or tenant-based participants, conducting regular face-to-face case management sessions. Assist clients with becoming involved in daily activities, scheduling and attending appointments, budgeting, socialization, health maintenance, sense of community, and progress in recovery. Educate client on the appropriate use of crisis intervention services versus 911 emergency calls, etc. Assist with the preparation of forms and any other documentation required for initial housing voucher approval with the Housing Authority (HACLA, LACDA, Brilliant Corners), including annual recertification thereafter. Provide education on topics pertaining to successful housing retention, decrease social isolation, and prevent relapse risks. This includes setting up and transporting clients on various field trips out into the community. Support clients by teaching and modeling independent living skills including money management and budgeting, housekeeping, nutrition and food preparation, appointment scheduling, health maintenance, social/recreational activities, and job training/placement (when appropriate) that will help them maintain independent living. Develop and implement an individualized service plan, in collaboration with each participant emphasizing coordinated strategies for addressing short and long-term housing needs, financial stability, educational, mental health, substance abuse issues, relapse prevention, medication management, vocational training, immigration, legal issues, and any other risk factors that could impede permanent housing of self-sufficiency. The service plan shall address the clients' needs, goals, steps to reach goals, timeframes for completing goals, and disposition of each goal as it is met or changed. Follow-up with each client on a regular basis to ensure they are making progress towards their goals Assist clients with maintaining medication and treatment regimens including accompanying/transporting clients to appointments in the community with health, mental health, and other care providers. Tia Turner, CEO Provide eviction prevention counseling; work with property management and HFH partner to help clients resolve issues that threaten their housing stability including tenant rights and responsibilities. Maintain confidentiality of Client information per HIPAA and internal agency guidelines. Maintain a current, thorough knowledge of community resources and utilize them to provide comprehensive, wrap-around services to clients. Monitor the resident's level of participation by providing wellness and safety checks as needed, such as when appointments are missed and frequency of engagement changes. Provide crisis intervention strategies when needed to promote the wellbeing of participants. QUALIFICATIONS AND REQUIREMENTS
Bachelor's degree preferred; or a minimum two years' experience working with homeless individuals, preferably those living with mental illness and/or substance addictions, or an equivalent combination of education and experience. In lieu of a degree, relevant work experience demonstrating acceptable knowledge and transferable skills will be considered. Experience and familiarity using HMIS (homeless database) and CES. Have expertise in the following areas: chronic homelessness, outreach and engagement strategies, intensive case management services, best practice models, mental health and substance use disorder services, crisis intervention, suicide assessment and prevention, affordable housing and public benefits applications, housing and landlord/tenant rights, eviction prevention, etc. Flexibility is required regarding scheduling and prioritizing tasks Ability to utilize critical thinking skills in decision-making situations, good organizational and record-keeping skills, and good independent judgment Valid California Class C Driver's license, reliable automobile, auto insurance, and an acceptable driving record. TRAVEL REQUIREMENTS
This position requires frequent visits to properties, attendance to on-site vendor meetings, community meetings, funding workshops, trainings, and other events in the greater Los Angeles County. Occasional attendance at events outside Los Angeles County may also be required.
