Brevard County, FL About the Role Aging Matters in Brevard is seeking a reliable and detail-oriented Client Based Services Representative to support our Brevard Senior Nutrition Program. This position serves as a critical communication hub, collecting, maintaining, and sharing information about Seniors at Lunch (SAL) and Meals on Wheels (MOW) services with Area Supervisors, Volunteer Coordinators, and kitchen staff. You will be a key point of contact for clients, caregivers, and program partners, helping ensure seniors receive timely, accurate, and well-coordinated services. Key Responsibilities Client & Caller Communication Retrieve and record phone messages from clients, caregivers, and emergency contacts regarding meal delivery Assist with answering phones, taking messages, and transferring calls to appropriate staff, including Volunteer Coordinators and Area Supervisors Make bi-weekly calls to clients receiving frozen home delivered meals Communicate well with clients and volunteers to provide excellent customer service Data Entry & Recordkeeping Update and record new client information in the database, ensuring accuracy and consistency Perform data entry of new SAL clients into AIMS and eCIRTS as needed Generate daily Purchase Orders recording cancellations and resumed meal services Review and reconcile weekly meal counts with Volunteer Coordinators Conduct internal monitoring of MOW Purchase Orders and weekly Route Planning sheets for accuracy Referrals & Coordination Record and provide new referral information to Area Supervisors and track details on the referral log Create referrals for other agency services by gathering caller information and routing it to the appropriate service line manager or coordinator Work with SAL Site Managers and SCAT transportation to coordinate transportation for new applicants and existing clients Call SAL Site Managers for meal counts Managed Care Medicaid Support Receive Managed Care Medicaid authorizations, enter new client data into AIMS, and contact clients with meal delivery start dates Communicate with Managed Care Medicaid providers regarding client concerns or renewal authorizations Track termination dates for Managed Care Medicaid client authorizations and communicate updates as needed General Support Inform management of problems, complaints, and changes in program status Assist MOW volunteer drivers with non-response procedures, documentation, and follow-up Print and forward HDM Route Sheets and SAL documents to staff on a weekly basis Assist with monthly volunteer log sheets and coordinate office supply needs with field staff Cross-train with other Client Based Services Representatives to maintain knowledge of all duties Fulfill assigned monthly lock-up duty at the main office May conduct

screenings of potential clients once training is completed Perform other related duties as assigned Qualifications Education High school diploma or G.E.D. equivalent required Experience At least one (1) year of office experience operating a multi-line phone system required Experience working with vulnerable populations highly desired Skills & Requirements Ability to manage a multi-line telephone system Proficiency with administrative and computer procedures, including database navigation Strong customer service orientation and communication skills Ability to multitask in a fast-paced office environment Working Conditions This position is based in the agency office and involves extended periods of sitting at a computer and on the phone, as well as occasional standing, bending, and lifting up to 25 pounds. As a condition of employment, staff may be required to support emergency and disaster response efforts in accordance with agency and county directives.
