Job Description
JOB SUMMARY
The Client Care Coordinator assists the Home Health Services Manager with all activities of the Home Health department. This Coordinator is a high-energy team player with a positive attitude, excellent computer skills and strong interpersonal skills to interact with all types of people including seniors, persons with physical and or cognitive deficits, caregivers, volunteers, community partners, vendors and the public.DUTIES AND RESPONSIBILITIES
Performs administrative duties including but not limited to coordinating logistics and scheduling of Home Health Eastside and Westside staff by utilizing field visits, phone calls, staffing call outs, member cancellations and new service requests, keeping records, maintaining cell phone assignments, coordinating timesheets and mileage, creating reports using various software and excel spreadsheets, working with Outlook Calendar, email distribution lists, mailing lists and similar. The Coordinator must be able to use Outlook, Excel, Word and other Microsoft Office applications. Available to be on call for call outs after hours and over the weekend. Assists with implementing Care Plans for Members as approved by Provider agency. Documents any changes to Member's health or living situation that may affect level of service provided and notifies Case Manager of changes. Informs staff of all necessary changes in Member care plans, service activities or other situations that alter services provided. This may include but is not limited to: Member out of town, admitted to hospital or nursing home, any personal situation that places Member services on hold or alters services. Ensures that Member is contacted and determines convenient day and time for services to be delivered. Communicates with Home Health Aides (HHAs) and ensures all hours requested are honored if available. Prepares schedules, maintains attendance records and reconciles time sheets using the scheduling system. Assists HHS Manager with review of schedules and makes appropriate revisions. Communicates discrepancies with HHAs and ensures all corrections are made.- Ensures that all documentation and billable activities in the home are properly applied to the schedules each week and turned in by 8:00am every Monday for payroll and billing processing.
- Ensures all Client Activity information is current and up to date with Case Management documentation of the Member.
- Investigates and reports details to HHS of all complaints from Members or Home Health Aides in an effort to resolve each situation within the guidelines and policies of NCCOA.
- Assists the HHS Manager in creating and implementing bi-monthly meeting content with Home Health Aides for training, testing, open-floor discussion and keeping abreast of policies.
- Provides services to all types of people with the utmost courtesy, kindness, respect, caring and compassion at all times. Takes time to listen, be supportive and make each person feel valued.
- Complies with all NCCOA policies and procedures in all areas including but not limited to cleanliness and hygiene, behavior and conduct, attendance and illness, training and housekeeping.
- Complies with all regulations in current Employee Handbook.
- Maintains a current knowledge of the NCCOA Emergency Management Plan, their role in it, procedures and participates in all Fire Drills in compliance with all regulations.
- Performs other duties as directed by the HHS Mgr.
