Job Description

Located in Freeport, IL, NCS Youth Home is a residential treatment center working with female adolescents in a behavioral mental health setting. The goal of the treatment program is to help youth change behavior so a discharge placement of a group home, foster home, or family home is an option. Youth present with mental health diagnoses including: conduct disorder, bipolar disorder, depression, PTSD, oppositional defiant disorder, and other trauma-based disorders. Holding youth accountable for actions, helping in the development of coping skills, improving interpersonal skills, and empowering independence and confidence is the goal of the program. Caring for self and others are the two rules of the NCS program. The Client Services Coordinator works in cooperation with the Unit Therapists, Unit Coordinators, Office Coordinators, and external referral and community sources to provide service coordination for adolescent females in our Qualified Residential Treatment Center. Please apply only if you have a Bachelor's Degree in a social service/human service related field, as well as at least one year of experience in a residential setting. Qualifications for Client Services Coordinator position include: Must be at least 21 years of age. Must have a Bachelor's Degree in a social service/human services related field Minimum one year of experience in a residential setting. Ability to work as a member of a team. Ability to set limits and to intervene when necessary, while maintaining a therapeutic rapport with the client. Ability to teach and role model effective communication skills and conflict resolution skills. Ability and willingness to be sensitive and responsive to the cultural diversity of all clients served. Must possess and maintain a valid driver's license and approval from the Illinois Department of Children and Family Services to transport children. This approval is based on employee's driving record. Primary responsibilities of the
Client Services Coordinator:
Attend ACR, court, mental health assessments, and other client-related appointments. Assist Registered Nurse with medical appointments Coordinate, or provide, transportation to client services off-campus Coordinate educational needs between client, school, referral sources Assist in completing assessments for mental health and treatment planning which pertains to current services and discharge Meet with youth on a weekly basis to provide counseling (wellness check-ins, educational support, skill-building, etc) Coordinate hair appointments, off-campus counseling, client school sports schedules, vocational appointments, any other client services in the community or off-campus Complete necessary paperwork/assessments required by DCFS and/or NCS (i.e.: IM-CANS, Ansel Casey, consents). Participate in weekly administrative meetings. Participate in weekly staff meetings, providing service coordination information, and assisting with any staff issues or concerns. Assist with communications to caseworkers/P.O.'s/approved family members. Assist with transitions and linkages to community services. Monitor the residents' community linkages and supports (i.e.: employment, education, AA/NA support groups). Help prepare for youth home visits. Participate in intake process through discharge planning of youth. Assist in 6 months of post discharge aftercare support of youth. Assist with staff scheduling. Assist with follow-up to incident reporting of youth. Complete Medicaid documentation in a timely manner. Help to maintain continuity and consistency from unit to unit and in implementation of treatment and agency policies between staff and units. Serve as on-call administrator on a rotating basis Serve as the back-up for other positions when necessary. Assume other duties as assigned.
