Dialysis Access Coordinator 1825 E Highway 34, Newnan, GA 30265 $47,017.45 - $56,623.17 a year - Full-time $47,017.45 - $56,623.17 a year - Full-time Overview The Dialysis Access Coordinator spearheads the end-to-end clinical coordination for patients with End-Stage Renal Disease (ESRD), managing the full lifecycle of dialysis access from initial evaluation and creation to ongoing maintenance and salvage. Acting as a high-impact liaison, this role integrates dialysis centers, referring providers, surgical teams, and patients to accelerate the transition from catheter dependence to permanent vascular access (AV fistula or graft), significantly reducing care delays and clinical complications. Key Responsibilities1. Patient Care Navigation & Advocacy Direct all patient navigation activities, serving as the primary advocate for ESRD patients navigating complex vascular access pathways. Orchestrate multidisciplinary logistics, including specialist consultations, diagnostic vein mapping, and comprehensive pre-operative clearances. Execute streamlined procedure scheduling across ASC, OBL, and hospital settings. Monitor and proactively manage the clinical continuum from catheter dependence through maturation to successful catheter removal. Enforce rigorous follow-up protocols post-intervention to ensure access longevity and patient safety. 2. Dialysis Center & Referral Network Management Cultivate and expand strategic partnerships with dialysis center leadership, nephrologists, and nursing staff. Audit dialysis center reports to identify high catheter prevalence and early indicators of access dysfunction. Drive business development by identifying new referral sources and capturing failing accesses to prevent "leakage" from the care system and region. 3. Operational Workflow & Performance Tracking Optimize multi-site scheduling workflows to maximize clinical efficiency and prioritize urgent thrombosed access cases. Manage the dialysis access registry and synthesize key program metrics to drive continuous improvement. Monitor and report on critical Key Performance Indicators (KPIs) including catheter dependence rates, time-to-access creation, and referral conversion rates. 5. Patient Education & Engagement Educate patients on: Importance of permanent access vs catheter Procedure expectations (AV fistula, AV graft, percutaneous AVF) Post-procedure care and warning signs Improve compliance with: Follow-up visits Scheduled procedures 6. Insurance Authorization & Documentation Support Coordinate prior authorizations for: Imaging (vein mapping, duplex) Procedures (fistulagram, angioplasty, thrombectomy, AV access creation) Ensure completeness of: Clinical documentation Referral records Work closely with billing/coding team to prevent delays 7. Program Development & Growth Support growth of the dialysis access service line by: Tracking referral sources and volumes Identifying gaps in care delivery Supporting outreach initiatives to dialysis centers Assist in implementing: Standard Operating Procedures (SOPs) Care pathways Quality improvement initiatives Qualifications Required Nursing (RN/LPN) OR Registered Medical Assistant OR Healthcare Coordinator experience Minimum 2 years experience in: Dialysis, vascular, interventional radiology, or cardiology setting Ability to travel to healthcare facilities Preferred Experience driving business development initiatives and physician outreach in targeted markets. Proven track record in improving KPIs such as catheter rate reduction and procedure turnaround times. Advanced proficiency with EMR systems and medical database management. Familiarity with EMR systems and scheduling platforms Knowledge of insurance authorization processes Strong organizational and multitasking skills Excellent communication skills with a compassionate approach to patient interactions.
