Job Description
Join our dedicated team as a Direct Care Professional , where your compassion and commitment will make a meaningful difference in the lives of individuals requiring support with daily activities and personal care. In this vital role, you will provide compassionate assistance, ensure safety, and promote independence for residents across various care settings. This position offers a rewarding opportunity to work closely with individuals with diverse needs, including those with developmental disabilities, dementia, or recovering from illness, in a supportive and dynamic environment. Assist residents with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support, including heavy lifting and the use of Hoyer lifts when necessary. Monitor residents' health by taking vital signs, observing patient conditions, and reporting changes to healthcare teams promptly. Administer medications following prescribed care plans while adhering to HIPAA regulations to protect patient confidentiality. Prepare nutritious meals and assist with meal planning and cooking to meet individual dietary needs. Maintain a clean and safe environment by performing routine cleaning tasks in residents' rooms and common areas. Support residents' emotional well-being through engaging activities, social interaction, and personalized care approaches tailored to each individual's preferences. Document care activities accurately in compliance with healthcare standards and facility policies, including patient monitoring logs and incident reports. Prior experience working in assisted living, nursing homes, home health, hospice care, or group homes is highly preferred. Knowledge of patient observation techniques, medication administration, and vital signs measurement. Certifications such as CPR and First Aid are required; additional training in Alzheimer's care, dementia care, memory care, or working with individuals with autism or disabilities is a plus. Ability to perform physical tasks including heavy lifting and the use of assistive devices like Hoyer lifts safely. Strong communication skills to effectively collaborate with team members and connect empathetically with residents. Familiarity with HIPAA regulations ensuring confidentiality of patient information.
