This role involves working with the individual in their home and community in order to maintain their independence, safety, and well-being. The ideal candidate will have a strong commitment to person-centered care and possess relevant experience in healthcare or social services. This role requires consistent attendance, punctuality, and professionalism. Our clients depend on routine and familiarity, and missed or late shifts have a real impact on their well-being. This position is not a good fit for individuals who frequently call out, arrive late, or are unable to commit to a given schedule. We are seeking team members who take responsibility seriously and are looking for long-term, dependable employment. Hours Mondays 5:30pm-8:30pm, Thursdays 10:00am-2pm Duties/Client Goals Teach independent living skills: assist individual with cooking, cleaning, and maintaining their apartment.
Teach money management skills:
assist individual in creating a budget, following their budget, making budget friendly choices while shopping, and paying bills on time.
Help individual find employment:
assist individual in filling out job applications, transport individual to interviews, help individual write their resume, practice interview skills. Teach positive communication skills Assist individual in participating in a hobby or class and increasing their involvement in community life. Maintain accurate documentation of care provided. Transport individual in the community. Mileage is reimbursed. Requirements Prior experience in working with people with developmental disabilities, social/human services, education, or health care is highly desirable. Must be able to pass a background check. Must be 18 years old or older with at least a GED or high school diploma. Must have a reliable vehicle with a valid driver's license and insurance. Must have a working smart phone or tablet. Ability to carefully and accurately follow staff action plans and safeguards. Must complete the paid, state-required online and in person trainings.
