Direct Support Professional

Job Overview Stepping Stones is seeking enthusiastic, reliable and caring individuals to work in the day support program to assist adults with intellectual, behavioral health and developmental disabilities. High school diploma/GED required. Valid driver's license and safe driving record include driving an 8-passenger van and 13 passenger van. Responsibilities include care of individuals in the day program, personal care of the individuals, and assistance with recreational, social and community activities. Assist with lunch preparation and eating with individuals daily. Experience with documentation of progress notes and individual plans. Experience with ID/DD individuals preferred. Responsibilities Assist individuals with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor patient health by taking vital signs, observing for changes, and reporting findings accurately while maintaining HIPAA compliance to protect client confidentiality. Support medication administration following prescribed care plans and ensuring proper documentation. Prepare nutritious meals aligned with dietary needs and assist with cooking to promote healthy living. Engage clients in social activities, recreational programs, and community outings to foster socialization and independence. Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols. Collaborate with healthcare professionals, family members, and team members to develop and implement personalized care plans that meet each individual's unique needs. Skills Proven experience in group home settings, assisted living facilities, or home & community care environments. Computer skills preferred to document and write progress notes Certification in CPR, first aid, and medication administration; additional training in HIPAA regulations is preferred. Strong communication skills for working effectively with individuals with disabilities or developmental disabilities. Compassionate attitude with patience for working with seniors or individuals managing chronic conditions like dementia or Alzheimer's disease.
