Job Description

Essential functions, duties & responsibilities: $1,000 Sign On Bonus
  • Provides monitoring, supervision, assistance, skill development, and behavior support and intervention to individuals served to ensure their safety, health, and welfare according to their needs and service plans.
  • Assists with basic personal care and grooming, including bathing, dental and hair care, toileting, dressing, and clothing/laundry care to individuals as assigned, and according to the needs and requirements of the service plans.
  • Assists with household services essential to the individuals' health and comfort in the home, including the care of linens, furniture, and general household cleaning.
  • Assists with the preparation of nutritious meals. Assists with eating and drinking tasks as needed.
  • Transports individuals served to community sites and events for basic errands, appointments, shopping, recreation, social and leisure activity, and work-related activity.
Provides monitoring, supervision, assistance, skill development, and behavior support and intervention to individuals served at the community sites, events, and appointments according to their needs and service plans. Also, to include the occasional use of staff personal vehicle with the mileage reimbursement.
  • Completes medication administration and health-related activities as required and trained according to the individual's service plan, and according to the requirements of agency procedures and certification systems.
  • Completes other health-related activities, including the escort of individuals to medical, therapeutic, clinical, and counseling appointments as assigned.
  • Records medication administration and health-related activities according to the timelines and requirements of regulations and procedures.
  • Performs first aid for individuals served according to the immediate needs and practices as trained in certification courses.
  • Communicate with TRS Nursing Department as needed.
  • Provides the correct support documentation when using client petty cash as instructed by company policy.
  • Completes daily documentation for completion of medication administration and health-related activities as required by certification and agency procedures.
  • Completes daily documentation for completion of support, skill development, and billing systems as required by the individuals' service plans and agency procedures.
  • Completes incident reports and other reports as required by agency procedures. Reports all incidents and concerns to the appropriate supervisory and on-call staff per agency procedures.
  • Collaborates and communicates with team members, professionals, medical and clinical providers, supervisors, and coworkers to ensure ongoing engagement and cooperation with the service delivery model.
  • Follows agency protocols regarding confidentiality of information related to individuals served, coworkers, and agency business.
  • Acts as a role model for individuals served and demonstrates a positive image of the agency and individuals served to ensure community inclusion and engagement.
  • Monitors for Major Unusual Incidents (MUI's) and Usual Incidents (UI's) and takes immediate action to ensure the health, safety, and welfare of the individuals being supported.
Reports all MUI's/UI's properly as required by TRS Policy.
Qualifications and specific job requirements:
  • Must have High School Diploma or G. E. D.
  • Must have all certifications required for employment by DODD and must maintain current certification.
  • Must be able to lift 50 pounds and perform physical assistance and intervention tasks.
  • Must maintain driving record according to insurance and regulatory requirements, if applicable.
  • Must possess basic skills for following written and verbal instructions, and for communicating client and job-related issues orally and in writing. Must be willing to work a flexible schedule as assigned to ensure the needs of the individuals are met.
  • Must adhere to all HIPAA regulations at all times.
  • Adheres to all local, state, and federal laws and TRS Policies and Procedures.
