This is an opportunity to lead a collaborative hospice team while remaining meaningfully connected to patient care. This role provides leadership and support to hospice clinicians while partnering closely with operations and executive leadership on strategic growth, quality initiatives, compliance, staffing, and patient care outcomes. The DCS maintains a small hospice caseload (approximately 4-5 patients) to remain actively engaged with patients, families, and frontline team needs.
Key Responsibilities Clinical Leadership & Team Support:
Lead, coach, and support a team of hospice RNs and CNAs Serve as a clinical resource for symptom management, complex care planning, and end-of-life care decisions Foster a positive team culture focused on compassion, accountability, collaboration, and excellence Participate in an on-call leadership rotation (approximately 5 days per month) Maintain a small patient caseload to stay connected to clinical care
Quality & Regulatory Oversight:
Ensure compliance with Medicare hospice Conditions of Participation and all state and federal regulations Lead quality assurance, infection control, and QAPI initiatives Support survey readiness activities and plans of correction as needed Conduct chart audits and clinical documentation review to ensure quality and compliance
Interdisciplinary Collaboration:
Lead bi-weekly Interdisciplinary Group (IDG) meetings Collaborate with physicians, intake staff, social workers, chaplains, and leadership to support coordinated patient care Partner with leadership on clinical operations, staffing, and process improvement initiatives
Staff Development & Operational Leadership:
Support hiring, onboarding, training, and mentorship of clinical staff Provide ongoing coaching, competency validation, and performance management Assist with operational oversight related to clinical supplies, DME, pharmacy coordination, and resource management Contribute to policy development and organizational growth initiatives
Qualifications Required:
Active Minnesota Registered Nurse (RN) license in good standing Minimum 3 years of hospice experience Minimum 2 years of hospice leadership, supervisory, or management experience Strong knowledge of hospice regulations, interdisciplinary care, and end-of-life best practices Excellent leadership, communication, organizational, and problem-solving skills Current CPR certification Valid driver's license, reliable transportation, and auto insurance
Preferred:
